Ketamine voor sedatie bij ernstig traumatisch hersenletsel (Ketamine)

Patiënten die voldoen aan de geschiktheidscriteria en toestemming hebben verkregen via LAR, zullen randomisatie ondergaan naar de interventie-arm met ketamine met propofol of naar de controle-arm volgens de standaardzorg.
Patiënten die deelnamen aan de interventiearm zullen propofol krijgen met een snelheid van 300 mcg/kg/uur (5 mcg/kg/min) met toevoeging van ketamine met een snelheid van 1000-5000 mcg/kg/uur (16,67 - 83,33 mcg/min). kg/min) (op basis van het gewicht op het moment van opname) met een extra bolus ketamine van 2 mg/kg (2000 mcg/kg), voor aanhoudende ICP-verhogingen van meer dan 22 mHg gedurende meer dan 5 minuten die niet aan andere oorzaken kunnen worden toegeschreven (hoesten enz.) . In de controlearm ontvangen patiënten hun institutionele analgosedatieprotocol. De sedatieprotocollen zullen worden voortgezet totdat de intracraniale drukmonitoring wordt opgeheven.
Dit onderzoek vindt plaats tijdens de ziekenhuisopname van de deelnemer. Het klinische team zal de behandeling uitvoeren volgens standaardpraktijken, inclusief alle beschikbare veiligheidsmaatregelen. Bijwerkingen zullen nauwlettend in de gaten worden gehouden en er kan worden besloten om de deelname van de proefpersoon aan het onderzoek stop te zetten als de gezondheid of veiligheid van de proefpersoon in gevaar komt.
Het onderzoeksteam zal één poliklinische follow-up uitvoeren nadat de onderzoeksinterventie is beëindigd.

Inclusiecriteria:
- Volwassenen van 18-65 jaar
- Ernstig traumatisch hersenletsel (TBI), met een Glasgow Coma Scale (GCS) ≤ 8, waarvoor intracraniale monitoring vereist is
- Plaatsing van intracraniale drukmonitor
- Behandeling ontvangen op een intensive care-afdeling (ICU)
Uitsluitingscriteria:
- Significante hart- en vaatziekten met recente coronaire interventie
- Zwangerschap
- Gevangenen
- Bekende allergie voor ketamine of propofol
