Intraosseus (IO) cefazoline en vancomycine bij primaire totale knieartroplastiek (TKA)
Intraossale vancomycine en cefazoline versus intraveneuze toediening bij primaire totale knieartroplastiek
Het doel van dit klinische onderzoek is het vergelijken van de werkzaamheid van intraveneuze (IV) en intraossale (IO) antibioticatoedieningstechnieken tijdens primaire totale knieartroplastiek (TKA) bij volwassenen die een TKA-procedure ondergaan in het Houston Methodist Hospital. De belangrijkste vragen die het wil beantwoorden zijn:
Beschermt IO-toediening van vancomycine en cefazoline tegen perioperatieve blootstellingsrisico's? Is er een verschil in postoperatieve complicaties tussen IV- en IO-toediening van deze geneesmiddelen?
Deelnemers worden gerandomiseerd om ofwel de standaardbehandeling IV-toediening van Vancomycine en Cefazoline te ontvangen, ofwel de IO-toediening van Vancomycine en Cefazoline.

Beschrijving
Inclusiecriteria:
- De patiënt zal een electieve primaire totale knieartroplastiek ondergaan.
- De patiënt is in staat het onderzoeksontwerp en de interventie te begrijpen en geeft geïnformeerde toestemming om aan het onderzoek deel te nemen.
- Patiënt is 18 jaar of ouder.
Uitsluitingscriteria:
- Contra-indicatie voor het ontvangen van vancomycine of cefazoline.
- Body mass index (BMI) > 40.
- Ongecontroleerde diabetes (gedefinieerd als A1c > 7,5%).
- De patiënt heeft binnen 7 dagen voorafgaand aan de geplande procedure IV vancomycine of cefazoline ontvangen of zal dit naar verwachting krijgen.
- Elke hardware, aandoening of anatomische status die verhindert dat de tibiale tuberkel een levensvatbare intraossale injectieplaats is.
- Weigering om deel te nemen
- Elke omstandigheid, naar het oordeel van de hoofdonderzoeker, die de deelnemer ongeschikt acht voor deelname aan het onderzoek.

Experimenteel: Intraossale toediening van vancomycine en cefazoline
Intraossaal vancomycine plus cefazoline wordt toegediend via een intraosseus canulatieapparaat (Arrow EZ-IO; Teleflex, Morrisville, NC) in de OK nadat het been steriel is voorbereid en is afgedekt voorafgaand aan de huidincisie en nadat de tourniquet is opgeblazen.
De injectie zal plaatsvinden in de tibiale tuberkel, een bekende veilige IO-injectieplaats.
De IO-injectie omvat 500 mg vancomycine en een dosis van 1 g cefazoline, die eerder werd gebruikt in eerdere IO-onderzoeken met cefazoline.

Actieve vergelijker: Standaard IV-toediening van vancomycine en cefazoline
Patiënten zullen het preoperatieve antibioticaregime voor orthopedische chirurgie van het Houston Methodist Hospital ontvangen voor patiënten met een primaire totale knieartroplastiek.
Dit omvat IV-antibiotica. Cefazoline zal worden gestart in de preoperatieve periode, ongeveer 1 uur vóór de incisie.
De dosis cefazoline is over het algemeen 2 g als de patiënt minder dan 120 kg weegt, en 3 g als de patiënt meer dan 120 kg weegt, in overeenstemming met de richtlijnen.
Vancomycine IV-toediening zal gebaseerd zijn op het gewicht van ongeveer 15 mg/kg, doorgaans 1000-1750 mg in 500 ml NS.
