Doel van de werkzaamheden:
Het doel van deze studie is om het erector spinae-blok te vergelijken met het transversus abdominus-vlakblok bij laparoscopische nefrectomie met betrekking tot de analgetische werkzaamheid en het postoperatieve morfineverbruik.
Statistische analyse
I. Steekproefomvang:
De steekproefomvang werd berekend met behulp van G-power-software. Uit een eerder onderzoek (referentie) bleek dat de hoeveelheid morfine die in de eerste 6 uur werd gebruikt bij patiënten die een TAP-blokkade kregen bij nefrectomie 12,4 ± 8,4 was. Ervan uitgaande dat de hoeveelheid met ten minste 50% zal veranderen met het andere blok, een power van 80% en een alfafout van 0,5, zal de minimaal vereiste steekproefomvang 60 patiënten zijn (30 in elke groep). Om uitval te compenseren, verhogen we dit aantal naar 35 per groep. (15)
II. Statistische analyse:
Alle meetindexen worden uitgedrukt als gemiddelde ± SD/standaardfout van het gemiddelde of getal (%). Na analyse van de normaliteit van de gegevensverdeling zullen normaal verdeelde gegevens worden vergeleken met de onafhankelijke steekproef-t-test. Ongepaarde kwantitatieve variabelen zullen worden geëvalueerd door de Student t-test en variantieanalyse. De Mann-Whitney U-test zal worden gebruikt voor vergelijking tussen groepen, en de Wilcoxon-test met ondertekende rang voor vergelijking tussen verschillende tijdstippen binnen dezelfde groep. Intergroepsvergelijking van categorische variabelen zal worden uitgevoerd door middel van de chikwadraattoets. Waarden met een AP-waarde van minder dan 0,05 worden als statistisch significant beschouwd. Alle gegevens zullen statistisch worden geanalyseerd door statistici met behulp van het SPSS 16.0-softwarepakket (IBM Corp., Armonk, NY, VS).

Beschrijving
Inclusiecriteria:
- Leeftijd patiënt (>18 en <60)
- Beide geslachten
- Fysieke statusklasse I en II van de American Society of Anesthesiologists (ASA).
- Patiënten die een laparoscopische nefrectomie ondergaan.
Uitsluitingscriteria:
- Weigering van regionaal blok
- Patiënten met ongecontroleerde diabetes of hypertensie
- Patiënten met neurologische, psychologische stoornissen of patiënten met een gebrek aan medewerking
- Patiënten met anatomische afwijkingen op de injectieplaats, huidlaesies of wonden op de plaats waar de naald zou moeten worden ingebracht.
- Patiënten met bloedingsstoornissen gedefinieerd als (INR >2) en/of (aantal bloedplaatjes <100.000/μl)
- Patiënten met een leverziekte, b.v. levercelfalen of levermaligniteit of leververgroting.
- Patiënten die allergisch zijn voor lokale anesthetica van amide.
- Gevallen die zijn omgezet in open chirurgie zullen ook van het onderzoek worden uitgesloten

Actieve vergelijker: Subcostal TAP Block
Group A, The patient was in the supine position then; the Subcostal TAP block was given by a high frequency linear ultrasound transducer (Siemens acuson x300 3-5MHz ultrasound).
After skin preparation and isolation, the transducer was placed 2 cm sub-xiphoid, then moved along the subcostal edge to identify the rectus abdominis muscle and the transversus abdominis then, a blunted tip, 20-gauge, short bevel needle (Pajunk Sonoplex, Geisingen, Germany) was introduced in-plane 2-3 cm lateral to the transducer, under direct ultrasound visualization.
After confirming the correct placement of the needle and the negative aspiration probe, the rest of the anaesthetic substance was injected along the subcostal line in the transversus abdominis plane 20 ml 0.25% bupivacaine on each side after aspiration to avoid intravascular placement, and the dissection of the plane was observed.
The block was performed bilaterally.

Group B, the patient was placed in the prone position.
Then, the Erector Spinae block was given by same ultrasound transducer .
It was sagittaly placed against the target vertebral level (T7 transverse process) in the prone position and moved in approximately 3-cm lateral to the spinous process..
The Erector Spinae muscle and transverse process was identified, and a same blunted tip , 20-gauge, short bevel needle was advanced, using the in-plane approach, in cephalad-to-caudal direction, through the interfascial plane between the Erector Spinae and the underlying transverse process under strict aseptic precautions until the tip is deep to erector spinae muscle, as evidenced by visible hydro-dissection below the muscle plane.
The block was performed bilaterally by injecting 40 mL of 0.25% bupivacaine (20 mL into each side) into the fascial plane between the deep surface of the Erector Spinae muscle and the transverse processes of the thoracic vertebrae laterally (specifically at T7).
