Therapeutisch effect van neuromodulatie op angststoornissen door high-definition transcraniële elektrische stimulatie
High-definition transcraniële elektrische stimulatie (HD-tES) is een niet-invasieve hersenneuromodulatietechniek die een kleine elektrische stroom op de hoofdhuid toepast om de neurale prikkelbaarheid te veranderen en gelokaliseerde hersenactivatie te stimuleren. Eerdere klinische onderzoeken hebben het gebruik van HD-tES onderzocht voor de behandeling van psychische aandoeningen zoals depressie, angst, obsessief-compulsieve stoornis en posttraumatische stressstoornis. Deze proef heeft tot doel de werkzaamheid en veiligheid van HD-tES te onderzoeken bij het verlichten van angstsymptomen bij patiënten met gegeneraliseerde angststoornis (GAS), waardoor het potentieel ervan als behandeling voor angststoornissen wordt gevalideerd.
Deelnemers worden willekeurig toegewezen aan een van de vier HD-tES-behandelingsgroepen: (1) HD-tES-remmende golfvorm (cDC+cTBS) toegepast op de rechter dorsolaterale prefrontale cortex (DLPFC) gedurende 10 minuten, gevolgd door een schijn-exciterende golfvorm (aDC+ iTBS)-stimulatie toegepast op de linker DLPFC gedurende 10 minuten. (2) Schijnremmende golfvorm (cDC+cTBS) stimulatie toegepast op de rechter DLPFC gedurende 10 minuten, gevolgd door HD-tES exciterende golfvorm (aDC+iTBS) toegepast op de linker DLPFC gedurende 10 minuten. (3) HD-tES-remmende golfvorm (cDC+cTBS) toegepast op de rechter DLPFC gedurende 10 minuten, gevolgd door HD-tES exciterende golfvorm (aDC+iTBS) toegepast op de linker DLPFC gedurende 10 minuten. (4) Schijn-remmende golfvormstimulatie (cDC+cTBS), toegepast op de rechter DLPFC gedurende 10 minuten, gevolgd door schijn-exciterende golfvormstimulatie (aDC+iTBS), toegepast op de linker DLPFC gedurende 10 minuten. Ongeacht de groepsopdracht zullen de deelnemers gedurende een periode van twee weken behandelsessies ondergaan, met vijf sessies per week en niet meer dan één sessie per dag. Elke sessie duurt ongeveer 20 minuten. Evaluaties zullen plaatsvinden vóór de behandeling, wekelijks tijdens de behandelingsperiode (aan het einde van de eerste en tweede week) en een vervolgevaluatie zal een week na het einde van de behandeling worden uitgevoerd.

Inclusiecriteria:
- Leeftijd tussen 18 en 65 jaar.
- Gediagnosticeerd met gegeneraliseerde angststoornis (GAD) door een psychiater volgens DSM-5-criteria.
- Hamilton Anxiety Rating Scale (HAM-A)-score ≥ 14.
- Hamilton Depression Rating Scale (HAM-D; versie met 17 items) score ≤ 17.
- Heeft gedurende ten minste 6 weken voorafgaand aan de inschrijving consequent psychologische begeleiding en/of medicatie ontvangen met stabiele typen en doseringen; of ongeschikt wordt geacht voor medicatie en/of psychologische begeleiding; of weigert medicatie en/of psychologische begeleiding.
Uitsluitingscriteria:
- Binnen 2 weken voorafgaand aan het onderzoek en tijdens de onderzoeksperiode rTMS of een andere vorm van niet-invasieve hersenstimulatietechnieken hebben ontvangen.
- Aanwezigheid van ernstige neurologische stoornissen (bijv. beroerte, hersentumor, epilepsie, organische hersenziekten) of psychiatrische stoornissen (bijv. schizofrenie en andere psychotische stoornissen, bipolaire stoornis, obsessief-compulsieve stoornis, andere soorten angststoornissen, middelenmisbruik).
- Ernstige of onstabiele fysiologische aandoeningen die het autonome of centrale zenuwstelsel kunnen aantasten (bijvoorbeeld acute gastro-intestinale ziekten, hart- en vaatziekten, schildklieraandoeningen).
- Geschiedenis van hartritmestoornissen.
- Aanwezigheid van geïmplanteerde medische elektronische apparaten (bijv. pacemakers).
- Aanwezigheid van metalen implantaten in het hoofd- of nekgebied.
- Open wonden op de hoofdhuid op de plaats van contact met de elektrode.
- Geschiedenis van hoofdchirurgie of aanzienlijk hoofdtrauma waardoor het individu, op basis van beoordeling door een arts, ongeschikt is voor opname.
- Individuen met een aanzienlijk zelfmoordrisico (HAM-D Item 3-score op zelfmoordrisico ≥ 3).
- Aanwezigheid van immuunstoornissen (bijv. systemische lupus erythematosus, reumatoïde artritis, spondylitis ankylopoetica, inflammatoire darmziekten).
- Personen met een abnormale of verhoogde gevoeligheid voor elektrische stimulatie, waardoor ze deze niet kunnen verdragen.
- Zwangerschap (voor vrouwelijke deelnemers: moet postmenopauzaal zijn of operatief gesteriliseerd. Vrouwen die zwanger kunnen worden, moeten een negatieve zwangerschapstest ondergaan. Vrouwelijke deelnemers die zwanger kunnen worden en hun mannelijke partners met vrouwelijke partners die zwanger kunnen worden, moeten akkoord gaan met het gebruik van effectieve anticonceptie tijdens de proef en gedurende vier maanden na de laatste onderzoeksinterventie, zoals orale anticonceptiva, methoden met dubbele barrière of spiraaltjes, of ermee instemmen om zich gedurende deze periode te onthouden van seksuele activiteit. Niet-vruchtbare vrouwen worden gedefinieerd als vrouwen die een bilaterale ovariëctomie hebben ondergaan of postmenopauzaal zijn).
- Het nemen van medicijnen die de aanvalsdrempel verlagen.
- Alcohol- of middelenmisbruik.
- Convexiteitsschedeldefecten of verhoogde intracraniale druk.
- Vrouwen die borstvoeding geven.
- Andere aandoeningen die op basis van beoordeling door een arts ongeschikt worden geacht voor transcraniële elektrische stimulatie.
