Beschrijving
Inclusiecriteria:
- Patiënten die niet -gearisteerde colonoscopie ondergaan, komen in aanmerking voor opname in deze studie.
- 18 tot 75 jaar oud.
- Voldoen aan de vereisten voor colonoscopie, met een goede cardiopulmonale functie en stabiele vitale tekenen.
- Een pre-procedurele visuele analoge schaal (VAS) pijnscore van minder dan 3.
Uitsluitingscriteria:
- Deelnemers met spraak- of cognitieve stoornissen.
- Degenen met acute anale of rectale stenose, acute perianale of rectale infecties, acute diverticulitis of actieve inflammatoire darmaandoeningen.
- Vrouwen die menstrueren, zwanger zijn of borstvoeding geven.
- Patiënten met actieve tuberculose, hemofilie of gevorderde kwaadaardige tumoren.
- Personen met sensorische verstoringen, geïmplanteerde pacemakers of defibrillators of enkelhuidzweren.
- Degenen die sedativa of analgetica hebben gebruikt, hetzij op lange termijn of binnen de afgelopen 24 uur.
- Personen met elke aandoening die interfereert met EEG-FNIRS-signaalverwerving, zoals schedelafwijkingen, geïmplanteerde metaal of elektronische apparaten, epilepsie of neurologische aandoeningen.

Experimenteel: Elektrische stimulatiegroep
Volgens de WAA -theorie zijn de geselecteerde onderste zone 1 (LZ1) en lagere zone 2 (LZ2) als volgt gedefinieerd: LZ1 bevindt zich tussen de mediale achillespees en de mediale malleolus, terwijl LZ2 is geplaatst aan de centrale binnenrand van de enkelgewricht, nabij de achterste rand van het scheenbeen.
Twee elektrodekussentjes worden op elke LZ1 en LZ2 van beide benen geplaatst, in totaal acht elektrodekussentjes.
Het Tens-WAA-apparaat wordt ingesteld op een frequentie van 2 Hz, een pulsbreedte van 500 μs en een continue golfvorm.
Deelnemers aan de stimulatiegroep ontvangen elektrische stromen bij hun maximale aanvaardbare intensiteit onder de pijndrempel.

In de elektrische stimulatiegroep wordt de huidige intensiteit van het apparaat aangepast aan de maximale tolerantie onder de pijndrempel van de deelnemer, terwijl in de controlegroep de huidige intensiteit op het minimum wordt ingesteld.

Placebo-vergelijker: controlegroep
Deelnemers aan de controlegroep hebben de elektroden in dezelfde posities geplaatst als die in de stimulatiegroep, met identieke elektrische stimulatiefrequentie en pulsbreedte -instellingen.
De huidige intensiteit wordt echter ingesteld op het minimumniveau.

In de elektrische stimulatiegroep wordt de huidige intensiteit van het apparaat aangepast aan de maximale tolerantie onder de pijndrempel van de deelnemer, terwijl in de controlegroep de huidige intensiteit op het minimum wordt ingesteld.

Neurovascular coupling is quantified from synchronized electroencephalography (EEG) and functional near-infrared spectroscopy (fNIRS) recordings.
EEG activity is decomposed into predefined delta, theta, alpha, and beta frequency bands, and band-specific power envelopes are coupled with fNIRS-derived oxygenated haemoglobin (HbO) signals within predefined cortical regions of interest.
A 10-second lag is applied to account for the delayed haemodynamic response following neural activity.
Coupling is estimated using a general linear model.
The peak β coefficient within the predefined analysis window is used to quantify maximum EEG-fNIRS neurovascular coupling strength.
Values are calculated separately for the predefined frequency bands and cortical regions of interest and compared between the distal transcutaneous electrical nerve stimulation and sham stimulation groups.

Pain intensity is assessed using a 10-cm Visual Analog Scale (VAS), ranging from 0 to 10, where 0 indicates no pain and 10 indicates the worst imaginable pain.
Participants report their pain intensity at three predefined procedural stages during colonoscopy: passage of the endoscope through the rectosigmoid junction, splenic flexure, and hepatic flexure.

EEG-Derived Cortical Neural Activity

Cortical neural activity is assessed using electroencephalography (EEG) during predefined stages of colonoscopy.
After signal preprocessing and artefact removal, EEG activity is analysed in four predefined frequency bands: delta, theta, alpha, and beta.
Outcome measures include relative spectral power, reflecting frequency-specific cortical activity, and pairwise EEG coherence, reflecting functional synchronization between cortical regions.
Stage-specific changes relative to the corresponding resting-state reference and between-group differences are evaluated to characterize alterations in cortical neural activity and network organization associated with procedural visceral pain and distal transcutaneous electrical nerve stimulation.
