Vibratietherapie en IASTM bij Subacromiaal Impingement Syndroom
De effecten van vibratietherapie en instrument-ondersteunde mobilisatie van zachte weefsels op pijn, bewegingsbereik, functionaliteit, gewrichtspositiegevoel en kwaliteit van leven bij personen met subacromiaal impingementsyndroom
Deze gerandomiseerde gecontroleerde klinische studie heeft tot doel de effecten van vibratietherapie en instrument-ondersteunde zachte weefselmobilisatie (IASTM), wanneer toegevoegd aan conventionele fysiotherapie, te onderzoeken op pijn, bewegingsbereik, proprioceptie, functionele status en kwaliteit van leven bij personen met de diagnose subacromiaal impingement syndroom (SAIS). SAIS is een van de meest voorkomende oorzaken van schouderpijn en wordt vaak geassocieerd met verstoorde scapulaire kinematica, rotator cuff-disfunctie, stijfheid van zachte weefsels en verminderde schoudermobiliteit.
In totaal zullen 48 deelnemers van 40-60 jaar met MRI-bevestigde SAIS willekeurig worden verdeeld in drie groepen: (1) Vibratietherapie + Conventionele Fysiotherapie, (2) IASTM + Conventionele Fysiotherapie, en (3) Conventionele Fysiotherapie (Controle). Alle interventies worden drie keer per week gedurende vier weken uitgevoerd. Uitkomstmaten omvatten pijnintensiteit (VAS), schouderbewegingsbereik (Goniometer Pro-app), proprioceptie bij 60° flexie en abductie, functionele status (DASH) en kwaliteit van leven (RC-QoL).
De studie heeft tot doel te bepalen of het toevoegen van vibratietherapie of IASTM aan standaard fysiotherapie extra kortetermijnvoordelen biedt bij het verminderen van pijn, verbeteren van gewrichtsmobiliteit, verhogen van proprioceptieve scherpte en vergroten van functionele capaciteit bij personen met subacromiaal impingement syndroom.

Het subacromiaal impingement syndroom (SAIS) is een van de meest voorkomende aandoeningen van het bewegingsapparaat, die ongeveer 20-30% van de algemene bevolking treft en een belangrijke oorzaak van schouderpijn vormt. SAIS is geassocieerd met veranderde scapulaire kinematica, disbalans van de rotatorcuffspieren, strakheid van weke delen, houdingsafwijkingen en verminderde neuromusculaire controle. Conventionele fysiotherapie-inclusief therapeutische oefeningen, stretchen, elektrotherapie modaliteiten en manuele technieken-wordt vaak gebruikt in de klinische praktijk. Echter, bewijs suggereert dat het combineren van manuele of op weke delen gebaseerde interventies met standaard fysiotherapie effectievere verbeteringen kan opleveren in pijn, mobiliteit en schouderfunctie.
Instrument-ondersteunde mobilisatie van weke delen (IASTM) is een manuele therapietechniek die gebruikmaakt van ergonomisch ontworpen roestvrijstalen instrumenten om gecontroleerde mechanische stimulatie aan weke delen te geven. IASTM heeft recentelijk aandacht gekregen vanwege het vermogen om de rekbaarheid van weke delen te vergroten, myofasciale release te bevorderen en het bewegingsbereik van gewrichten te verbeteren. Evenzo is aangetoond dat trillingsgebaseerde interventies-inclusief percussiemassagetherapie-de circulatie verbeteren, pijn verminderen, spierstijfheid verminderen en de mobiliteit verbeteren door mechanische oscillatie en stimulatie van sensorische receptoren. Ondanks hun groeiende populariteit bestaat er beperkt bewijs met betrekking tot de directe vergelijking van trillingstherapie en IASTM bij personen met SAIS.
Deze gerandomiseerde gecontroleerde studie heeft tot doel te evalueren of het toevoegen van trillingstherapie of IASTM aan conventionele fysiotherapie superieure resultaten oplevert in vergelijking met alleen conventionele fysiotherapie. In totaal zullen 48 volwassenen van 40-60 jaar met MRI-bevestigd SAIS willekeurig worden verdeeld in drie groepen: (1) Trillingstherapie + Conventionele Fysiotherapie, (2) IASTM + Conventionele Fysiotherapie, en (3) Conventionele Fysiotherapie (Controle). Alle interventies zullen drie keer per week gedurende vier weken worden uitgevoerd. Trillingstherapie zal worden toegepast met een percussiemassageapparaat (33 Hz) op de deltaspier en rotatorcuffspieren langs het pad van oorsprong tot aanhechting. IASTM zal worden toegepast met gestandaardiseerde sweep- en borsteltechnieken onder een hoek van 45° op zowel voorste als achterste schouderspiergroepen. De controlegroep zal conventionele fysiotherapie ontvangen bestaande uit echografie, TENS, cryotherapie, passief stretchen, Codman-oefeningen, stok-oefeningen, vingerladderoefeningen en progressieve krachttraining, onder begeleiding van een fysiotherapeut.
Uitkomstmaten omvatten pijnintensiteit (VAS), schouderbewegingsbereik gemeten met de Goniometer Pro mobiele applicatie, proprioceptie beoordeeld bij 60° flexie en abductie, functionele beperking geëvalueerd met de DASH-vragenlijst, en schoudergerelateerde kwaliteit van leven gemeten met de RC-QoL-schaal. Beoordelingen zullen worden uitgevoerd bij aanvang en na de 4-weken durende interventieperiode.
Deze studie probeert bij te dragen aan de huidige literatuur door twee veelgebruikte interventies voor weke delen-trillingstherapie en IASTM-rechtstreeks te vergelijken binnen de SAIS-populatie. De bevindingen worden verwacht clinici te informeren over de potentiële aanvullende voordelen van het combineren van deze modaliteiten met conventionele fysiotherapie om pijnvermindering, functionele verbetering en neuromusculaire uitkomsten bij personen met het subacromiaal impingement syndroom te optimaliseren.

- Mannen en vrouwen tussen de 40 en 60 jaar
- Aanwezigheid van symptomen gedurende meer dan een maand
Exclusiecriteria:
- Geschiedenis van een operatie aan de aangedane arm
- Aanwezigheid van een open wond aan de arm
- Bestaande infectie
- Ontstekingsziekte van de gewrichten
- Schouderartrose
- Aanwezigheid van reumatische aandoening

Experimenteel: Percussion Massage Therapy Plus Conventional Physiotherapy
Participants received the standardized conventional physiotherapy programme for four weeks and additionally received percussion massage therapy three times per week for four weeks.

Percussion massage therapy was administered using a handheld Hypervolt device (Hyperice, California, USA) at 33 Hz, corresponding to level 2 of the device. A flat-head attachment was used to provide uniform contact with the treated tissues. The intervention was applied to the deltoid, supraspinatus, infraspinatus, and teres minor muscles along their anatomical fibre orientation. Each muscle was treated for three minutes using controlled longitudinal strokes applied alternately in proximal-to-distal and distal-to-proximal directions. The intervention was administered by the same licensed physiotherapist three times per week for four weeks, concurrently with the conventional physiotherapy programme.

Change in Shoulder Joint Position Sense Error

|
Shoulder joint position sense was assessed using a digital goniometer at target positions of 60 degrees of shoulder flexion and 60 degrees of shoulder abduction. The affected shoulder was passively positioned at the target angle, and the participant was allowed up to 10 seconds to memorize the position. The arm was then returned to the neutral position, and the participant was asked to actively reproduce the target position with the eyes closed. The absolute angular difference between the target position and the reproduced position was calculated in degrees. Three trials were conducted for flexion and three trials for abduction, and the mean absolute error for each direction was used in the analysis. Lower values indicate greater joint position sense accuracy. |
