Intermediaire versus standaard dosering Enoxaparine ter preventie van veneuze trombo-embolie bij ernstig getraumatiseerde patiënten: een multicenter dubbelblind gerandomiseerd gecontroleerd onderzoek
Venous trombo-embolie is een veelvoorkomend probleem bij patiënten met ernstig trauma. Richtlijnen voor de preventie van veneuze trombo-embolie omvatten het gebruik van farmacologische tromboprofylaxe, voornamelijk met laagmoleculair gewicht heparine, en/of mechanische tromboprofylaxe. Echter, er wordt een hoge incidentie van veneuze trombo-embolie waargenomen ondanks standaard dosering tromboprofylaxe (zoals enoxaparine 40 mg eenmaal daags).
Een verhoging van de dosering van antistollingsmiddelen zou de tromboprofylaxe bij traumapatiënten kunnen verbeteren. Tot op heden hebben twee gerandomiseerde onderzoeken het effect van op gewicht gebaseerde dosering van laagmoleculair gewicht heparine versus vaste dosering bij traumapatiënten beoordeeld. Deze pilotstudies toonden geen statistisch verschil tussen de groepen aan, hoewel er een trend was in het voordeel van een lagere incidentie van diepe veneuze trombosen met de verhoogde dosering laagmoleculair gewicht heparine profylaxe. Echter, beide studies omvatten patiënten met niet-ernstig trauma en de tweede studie richtte zich alleen op diepe veneuze trombosen. Andere onderzoeken suggereerden dat een hogere dan standaard dosering van laagmoleculair gewicht heparine, soms
geleid door de anti-Xa-activiteit, de incidentie van veneuze trombo-embolie bij ernstig trauma vermindert zonder bloedingsevenementen te verhogen, maar deze waren observationeel van aard.
De hypothese van de HEPTRAUMA-trial is dat, bij patiënten met ernstig trauma, een tromboprofylaxe met een intermediaire dosering laagmoleculair gewicht heparine (tweemaal de standaarddosis) de incidentie van grote veneuze trombo-embolie vermindert in vergelijking met standaarddosering.

Beschrijving
Inclusiecriteria:
- Alle volwassen patiënten (leeftijd ≥18 jaar) opgenomen op een intensive care-afdeling na trauma met een verwachte verblijfsduur >48 uur en een geplande toediening van LMWH.
- Aangesloten bij het Franse sociale zekerheidssysteem of Europees (CEAM)
Exclusiecriteria:
- Prehospitaal hartstilstand
- Ziekenhuisopname >72 uur
- Meer dan één dosis profylactisch antistollingsmiddel al toegediend sinds trauma
- Nierfalen gedefinieerd door een creatinineklaring van 30 mL/min of minder (Cockcroft-Gault-formule)
- Lichaamsgewicht >100 kg of <45 kg
- Indicatie voor therapeutische antistolling
- Grote bekende trombofilie (bijv. antifosfolipidensyndroom, antitrombinedeficiëntie)
- Constitutionele bloedingsstoornis (hemofilie, ziekte van von Willebrand, stollingsfactordeficiëntie, bloedplaatjesstoornis).
- Trombocytopenie lager dan 50 G.L-1
- Geschiedenis van heparine-geïnduceerde trombocytopenie
- Overgevoeligheid voor het studiegeneesmiddel
- Beperking van levensondersteuning, levensverwachting ≤7 dagen of palliatieve zorg
- Contra-indicatie voor toediening van antistollingsmiddel volgens de SPC (Actieve klinisch significante bloeding of een aandoening geassocieerd met een hoog risico op bloedingen, zoals recente hemorragische beroerte, maagzweer, aanwezigheid van een maligne tumor met hoog risico op bloedingen, recente hersen-, ruggengraat- of oogheelkundige operatie, bekende of vermoedelijke slokdarmvarices, arterioveneuze malformaties, vasculaire aneurysma's of grote intrarachidale of intracerebrale vasculaire afwijkingen.)

Actieve vergelijker: Standaard dosering LMWH
Patiënten in de controlegroep ontvangen gedurende 14 dagen een standaarddosis enoxaparine.
Zij zullen twee injecties per dag ontvangen, waarvan één een placebo is.

In de controlegroep krijgen patiënten 1 injectie met enoxaparine 4000 IE en 1 injectie met placebo tot dag 14 of ziekenhuisontslag in dezelfde vorm als enoxaparine (subcutane injectie). Een placebo-injectie wordt indien nodig toegediend om de verblinding te behouden en hetzelfde aantal injecties als in de intermediaire-dosisgroep te garanderen. |

Experimenteel: Intermediaire dosis LMWH
Patiënten in de experimentele groep ontvangen een intermediaire dosis enoxaparine.
Ze ontvangen twee injecties enoxaparine per dag.

In de experimentele groep zullen patiënten tot dag 14 of ziekenhuisontslag 2 injecties met enoxaparine 4000 IE ontvangen.

Analyses zullen worden uitgevoerd in de ITT-populatie met behulp van een concurrerend-risicobenadering (overlijden voor VTE wordt beschouwd als een concurrerende gebeurtenis).
