ACUITY-forsøket: En randomisert sammenligning av angiomax (bivalirudin) versus heparin (ufraksjonert heparin eller enoksaparin) hos pasienter som gjennomgår tidlig invasiv behandling for akutte koronare syndromer uten ST-segmentheving
Hensikten med denne studien er å vise at sammenlignet med heparin (enoksaparin eller ufraksjonert heparin) og rutinemessig GPIIb/IIIa-hemming (enten startet på forhånd eller på tidspunktet for perkutan koronar intervensjon [PCI]; Arm A):
- Bivalirudin med rutinemessig GPIIb/IIIa-hemming (enten startet på forhånd eller på tidspunktet for PCI; Arm B) gir ikke-inferiøre eller overlegne generelle kliniske resultater og
- Bivalirudin alene (arm C) reduserer klinisk signifikant blødning. Et viktig sekundærmål for denne sammenligningen er å vise at bivalirudin ikke er dårligere for iskemiske komplikasjoner.

Beskrivelse
Inklusjonskriterier:
- Være >=18 år på randomiseringstidspunktet.
- Har symptomer som inkluderer minst 10 minutter med angina eller angina ekvivalent som etterforskeren mener har stor sannsynlighet for å være iskemisk i opprinnelse innen 24 timer før randomisering i samsvar med en diagnose ustabil angina/ikke-ST-elevasjonsmyokardinfarkt (UA/ NSTEMI) (pasienter med symptomer som er atypiske for hjerteiskemi bør ikke meldes inn).
Oppfyll minst ett av følgende kriterier for UA/NSTEMI:
- Alle de følgende fire funksjonene: *Alder >= til 65 år; *aspirin tatt i løpet av de siste 7 dagene; *to eller flere episoder med angina de siste 24 timene; *tre eller flere av følgende risikofaktorer: hypertensjon, høyt kolesterol, familiehistorie, diabetes, nåværende røyker ELLER
- EKG-endringer: Ny eller antagelig ny ST-segmentdepresjon >=0,1 MV (>=1 mm), eller forbigående (=0,1 MV (>=1 mm) i minst 2 sammenhengende avledninger, ELLER
- Unormale hjerteenzymer innen 24 timer før registrering definert som forhøyet troponin I, T eller kreatinfosfokinase-MB isoenzym (CPK-MB) nivå høyere enn stedets øvre normalgrense (ULN) ELLER
- Anamnese med koronarsykdom med dokumentasjon på ett av følgende: *forutgående angiografi (koronarstenose >50%); *tidligere PCI- eller koronararteriebypassgrafting (CABG); *tidligere bestemt, dokumentert hjerteinfarkt.
- Gi skriftlig informert samtykke før oppstart av studierelaterte prosedyrer.
Ekskluderingskriterier:
- Forventet manglende evne til å utføre angiografi innen 72 timer etter randomisering og forventet manglende evne til å utføre nødvendig intervensjon (PCI eller CABG) innenfor indekssykehusinnleggelsen.
- ST-segmentheving på >1 mm i 2 sammenhengende EKG-avledninger som varer i >30 minutter, eller ny venstre grenblokk, eller et klinisk syndrom forenlig med akutt utviklende transmural MI som krever umiddelbar trombolytisk eller intervensjonell reperfusjonsbehandling.
- Kardiogent sjokk (systolisk blodtrykk 30 minutter som ikke reagerer på intravenøse væsker, eller krever intravenøse pressorer eller en intra-aorta ballongpumpe).
- Blødende diatese eller en historie med hemorragisk hjerneslag eller annen intracerebral blødning, cerebral arteriovenøs misdannelse, cerebral aneurisme eller tidligere iskemisk hjerneslag i løpet av de siste 2 årene, eller ethvert tidligere slag med gjenværende nevrologisk underskudd. Gastrointestinal eller genitourinær blødning i løpet av de siste 2 ukene.
- Antall blodplater
- Pasienter på warfarin eller fenprokumon, med mindre de trygt kan seponeres og baseline INR er < 1,5 ganger kontroll.
- Allergi mot svinekjøtt eller svinekjøttprodukter.
- Pasienter som har blitt startet på og mottatt 2 eller flere doser lavmolekylært heparin for gjeldende innleggelse før randomisering (pasienter som har fått én dose lavmolekylært heparin kan fortsatt være registrert.
- Pasienter som er startet på bivalirudin i løpet av 6 timer før randomisering
- Trombolytisk behandling eller bruk av abciximab i løpet av de siste 24 timene.
- Behandling med en GPIIb/IIIa-hemmer ved randomiseringstidspunktet, som ikke kan avbrytes.
- Pasienter på Arixtra (fondaparinux).
Absolutt kontraindikasjon eller allergi mot:
- hvilken som helst av enoksaparin, ufraksjonert heparin, bivalirudin eller aspirin
- både abciximab og eptifibatid
- både eptifibatid og tirofiban
- jodholdig kontrast som ikke kan premedisineres
- Kontraindikasjoner for angiografi, inkludert, men ikke begrenset til, alvorlig perifer vaskulær sykdom.
- Angina av sekundære årsaker.
- Graviditet eller ammende mødre. Kvinner i fertil alder må ha en negativ urin- eller serumgraviditetstest før påmelding.
- Beregnet serumkreatininclearance < 30 ml/min (bestemt av Cockcroft Gault-formelen: ((140-alder i år) x vekt i kg)/(814.464) x Kreatinin i mmol/l) eller /(72 x [Kreatinin i mg/dL]). For kvinner, multipliser med 0,85.
- Tidligere påmelding til denne studien.
- Pasienter som for tiden er registrert i en annen legemiddelstudie som ikke har fullført oppfølgingsfasen.
