Confronto tra Angiomax e eparina nelle sindromi coronariche acute (ACS)
Lo studio ACUITY: un confronto randomizzato tra Angiomax (bivalirudina) e eparina (eparina non frazionata o enoxaparina) in pazienti sottoposti a trattamento invasivo precoce per sindromi coronariche acute senza sopraslivellamento del segmento ST
Lo scopo di questo studio è dimostrare che, rispetto all'eparina (enoxaparina o eparina non frazionata) e all'inibizione di routine della GPIIb/IIIa (iniziata in anticipo o al momento dell'intervento coronarico percutaneo [PCI]; Braccio A):
- La bivalirudina con l'inibizione di routine della GPIIb/IIIa (iniziata in anticipo o al momento del PCI; braccio B) fornisce risultati clinici complessivi non inferiori o superiori e
- La sola bivalirudina (braccio C) riduce il sanguinamento clinicamente significativo. Un obiettivo secondario importante per questo confronto è mostrare che la bivalirudina non è inferiore per le complicanze ischemiche.

Descrizione
Criterio di inclusione:
- Avere un'età >=18 anni al momento della randomizzazione.
- Avere sintomi che includono almeno 10 minuti di angina o angina equivalente che lo sperimentatore ritiene abbia un'alta probabilità di essere di origine ischemica entro le 24 ore precedenti la randomizzazione coerente con una diagnosi di angina instabile/infarto del miocardio senza sopraslivellamento del tratto ST (UA/ NSTEMI) (i pazienti con sintomi atipici per ischemia cardiaca non devono essere arruolati).
Soddisfare almeno uno dei seguenti criteri per UA/NSTEMI:
- Tutte le seguenti quattro caratteristiche: *Età >= a 65 anni; *aspirina assunta negli ultimi 7 giorni; *due o più episodi di angina nelle ultime 24 ore; *tre o più dei seguenti fattori di rischio: ipertensione, colesterolo alto, storia familiare, diabete, attuale fumatore OPPURE
- Alterazioni dell'ECG: sottoslivellamento del tratto ST nuovo o presumibilmente nuovo >=0,1 MV (>=1 mm) o transitorio (=0,1 MV (>=1 mm) in almeno 2 derivazioni contigue, OPPURE
- Enzimi cardiaci anormali entro le 24 ore prima dell'arruolamento definiti come livelli elevati di troponina I, T o creatina fosfochinasi-MB (CPK-MB) superiori al limite superiore della norma (ULN) del sito OPPURE
- Anamnesi di malattia coronarica con documentazione di uno dei seguenti: *angiografia precedente (stenosi coronarica >50%); * PCI precedente o innesto di bypass coronarico (CABG); *pregresso infarto miocardico definito e documentato.
- Fornire il consenso informato scritto prima dell'inizio di qualsiasi procedura correlata allo studio.
Criteri di esclusione:
- Incapacità prevista di eseguire l'angiografia entro 72 ore dalla randomizzazione e incapacità prevista di eseguire qualsiasi intervento richiesto (PCI o CABG) nell'ambito del ricovero indice.
- Sopraslivellamento del segmento ST > 1 mm in 2 derivazioni ECG contigue di durata > 30 minuti, o nuovo blocco di branca sinistra, o una sindrome clinica compatibile con infarto del miocardio transmurale acuto in evoluzione che richieda una terapia trombolitica immediata o di riperfusione interventistica.
- Shock cardiogeno (pressione arteriosa sistolica per 30 minuti che non risponde ai fluidi per via endovenosa o che richiede agenti pressori per via endovenosa o una pompa a palloncino intra-aortico).
- Diatesi emorragica o qualsiasi storia di ictus emorragico o altro sanguinamento intracerebrale, malformazione artero-venosa cerebrale, aneurisma cerebrale o precedente ictus ischemico negli ultimi 2 anni o qualsiasi precedente ictus con deficit neurologico residuo. Sanguinamento gastrointestinale o genitourinario nelle ultime 2 settimane.
- Conta piastrinica
- Pazienti in trattamento con warfarin o fenprocumone, a meno che non possano essere interrotti in modo sicuro e l'INR al basale sia < 1,5 volte il controllo.
- Allergia alla carne di maiale o ai prodotti a base di carne di maiale.
- Pazienti che hanno iniziato e hanno ricevuto 2 o più dosi di eparina a basso peso molecolare per l'attuale ricovero prima della randomizzazione (i pazienti che hanno ricevuto una dose di eparina a basso peso molecolare possono ancora essere arruolati.
- Pazienti che hanno iniziato la terapia con bivalirudina nelle 6 ore precedenti la randomizzazione
- Terapia trombolitica o uso di abciximab nelle ultime 24 ore.
- Trattamento con un inibitore GPIIb/IIIa al momento della randomizzazione, che non può essere interrotto.
- Pazienti su Arixtra (fondaparinux).
Controindicazione assoluta o allergia a:
- uno qualsiasi tra enoxaparina, eparina non frazionata, bivalirudina o aspirina
- sia abciximab che eptifibatide
- sia eptifibatide che tirofiban
- mezzo di contrasto iodato che non può essere premedicato
- Controindicazioni all'angiografia, inclusa ma non limitata a grave malattia vascolare periferica.
- Angina da cause secondarie.
- Madri in gravidanza o allattamento. Le donne in età fertile devono avere un test di gravidanza su siero o urina negativo prima dell'arruolamento.
- Clearance calcolata della creatinina sierica < 30 mL/min (determinata dalla formula di Cockcroft Gault: ((140-età in anni) x peso in kg)/(814,464 x Creatinina in mmol/l) o /(72 x [Creatinina in mg/dL]). Per le donne moltiplicare per 0,85.
- Precedente iscrizione a questo studio.
- Pazienti attualmente arruolati in un altro studio sperimentale sui farmaci che non ha completato la fase di follow-up.
