Comparación de Angiomax versus heparina en síndromes coronarios agudos (SCA)
El ensayo ACUITY: una comparación aleatoria de Angiomax (bivalirudina) versus heparina (heparina no fraccionada o enoxaparina) en pacientes sometidos a tratamiento invasivo temprano para síndromes coronarios agudos sin elevación del segmento ST
El propósito de este estudio es mostrar que, en comparación con la heparina (enoxaparina o heparina no fraccionada) y la inhibición rutinaria de GPIIb/IIIa (ya sea iniciada por adelantado o en el momento de la intervención coronaria percutánea [ICP]; Grupo A):
- La bivalirudina con la inhibición rutinaria de GPIIb/IIIa (ya sea iniciada por adelantado o en el momento de la ICP; Grupo B) proporciona resultados clínicos generales no inferiores o superiores y
- La bivalirudina sola (Brazo C) reduce el sangrado clínicamente significativo. Un objetivo secundario importante para esta comparación es demostrar que la bivalirudina no es inferior para las complicaciones isquémicas.

Descripción
Criterios de inclusión:
- Tener una edad >=18 años en el momento de la aleatorización.
- Tienen síntomas que incluyen al menos 10 minutos de angina o angina equivalente que el investigador cree que tiene una alta probabilidad de ser de origen isquémico dentro de las 24 horas previas a la aleatorización consistente con un diagnóstico de angina inestable/infarto de miocardio sin elevación del segmento ST (AI/ NSTEMI) (no se deben incluir pacientes con síntomas atípicos de isquemia cardíaca).
Cumplir con al menos uno de los siguientes criterios para UA/NSTEMI:
- Todas las siguientes cuatro características: *Edad >= a 65 años; *aspirina tomada en los últimos 7 días; *dos o más episodios de angina en las últimas 24 horas; *tres o más de los siguientes factores de riesgo: hipertensión, colesterol alto, antecedentes familiares, diabetes, fumador actual O
- Cambios en el ECG: depresión del segmento ST nueva o presumiblemente nueva >=0,1 VM (>=1 mm) o transitoria (=0,1 VM (>=1 mm) en al menos 2 derivaciones contiguas, O
- Enzimas cardíacas anormales dentro de las 24 horas antes de la inscripción definidas como troponina I, T elevada o nivel de isoenzima de creatina fosfoquinasa-MB (CPK-MB) mayor que el límite superior normal (ULN) del sitio O
- Antecedentes de enfermedad arterial coronaria con documentación de uno de los siguientes: *angiografía previa (estenosis coronaria de >50%); *anterior PCI o injerto de derivación de la arteria coronaria (CABG); *Infarto de miocardio previo documentado y definitivo.
- Proporcione su consentimiento informado por escrito antes de iniciar cualquier procedimiento relacionado con el estudio.
Criterio de exclusión:
- Incapacidad anticipada para realizar una angiografía dentro de las 72 horas posteriores a la aleatorización e incapacidad anticipada para realizar cualquier intervención requerida (PCI o CABG) dentro de la hospitalización índice.
- Elevación del segmento ST de >1 mm en 2 derivaciones de ECG contiguas que duran >30 minutos, o nuevo bloqueo de rama izquierda del haz de His, o un síndrome clínico compatible con infarto de miocardio transmural en evolución aguda que requiere terapia de reperfusión intervencionista o trombolítica inmediata.
- Choque cardiogénico (presión arterial sistólica durante 30 minutos que no responde a líquidos intravenosos o requiere agentes presores intravenosos o un balón de contrapulsación intraaórtico).
- Diátesis hemorrágica o cualquier antecedente de accidente cerebrovascular hemorrágico u otra hemorragia intracerebral, malformación arteriovenosa cerebral, aneurisma cerebral o accidente cerebrovascular isquémico previo en los últimos 2 años, o cualquier accidente cerebrovascular previo con déficit neurológico residual. Sangrado gastrointestinal o genitourinario en las últimas 2 semanas.
- Recuento de plaquetas
- Pacientes que toman warfarina o fenprocumon, a menos que puedan suspenderse de forma segura y el INR inicial sea < 1,5 veces el control.
- Alergia al cerdo o productos derivados del cerdo.
- Los pacientes que comenzaron y recibieron 2 o más dosis de heparina de bajo peso molecular para la admisión actual antes de la aleatorización (los pacientes que recibieron una dosis de heparina de bajo peso molecular aún pueden inscribirse).
- Pacientes que han comenzado con bivalirudina en las 6 horas previas a la aleatorización
- Terapia trombolítica o uso de abciximab en las últimas 24 horas.
- Tratamiento con un inhibidor de GPIIb/IIIa en el momento de la aleatorización, que no se puede interrumpir.
- Pacientes en Arixtra (fondaparinux).
Contraindicación absoluta o alergia a:
- cualquiera de enoxaparina, heparina no fraccionada, bivalirudina o aspirina
- tanto abciximab como eptifibatida
- tanto eptifibatide como tirofiban
- contraste yodado que no puede ser premedicado
- Contraindicaciones para la angiografía, incluidas, entre otras, enfermedades vasculares periféricas graves.
- Angina por causas secundarias.
- Embarazadas o madres lactantes. Las mujeres en edad fértil deben tener una prueba de embarazo en orina o suero negativa antes de la inscripción.
- Aclaramiento de creatinina sérica calculado < 30 ml/min (Determinado por la fórmula de Cockcroft Gault: ((140-edad en años) x peso en kg)/(814.464 x Creatinina en mmol/l) o /(72 x [Creatinina en mg/dL]). Para las mujeres, multiplique por 0,85.
- Inscripción previa en este estudio.
- Pacientes actualmente inscritos en otro estudio de fármaco en investigación que no ha completado la fase de seguimiento.
