Vergleich von Angiomax versus Heparin bei akutem Koronarsyndrom (ACS)

Die ACUITY-Studie: Ein randomisierter Vergleich von Angiomax (Bivalirudin) versus Heparin (unfraktioniertes Heparin oder Enoxaparin) bei Patienten, die sich einer frühen invasiven Behandlung des akuten Koronarsyndroms ohne ST-Strecken-Hebung unterziehen
Der Zweck dieser Studie ist es zu zeigen, dass im Vergleich zu Heparin (Enoxaparin oder unfraktioniertes Heparin) und routinemäßiger GPIIb/IIIa-Hemmung (entweder vor Beginn oder zum Zeitpunkt der perkutanen Koronarintervention [PCI]; Arm A):
- Bivalirudin mit routinemäßiger GPIIb/IIIa-Hemmung (entweder im Vorfeld oder zum Zeitpunkt der PCI begonnen; Arm B) liefert nicht unterlegene oder überlegene klinische Gesamtergebnisse und
- Bivalirudin allein (Arm C) reduziert klinisch signifikante Blutungen. Ein wichtiges sekundäres Ziel dieses Vergleichs ist es zu zeigen, dass Bivalirudin bei ischämischen Komplikationen nicht unterlegen ist.

Beschreibung
Einschlusskriterien:
- Zum Zeitpunkt der Randomisierung ≥ 18 Jahre alt sein.
- innerhalb der 24 Stunden vor der Randomisierung Symptome aufweisen, die eine mindestens 10-minütige Angina pectoris oder ein anginales Äquivalent beinhalten, von denen der Prüfarzt annimmt, dass sie innerhalb von 24 Stunden vor der Randomisierung mit hoher Wahrscheinlichkeit ischämischen Ursprungs sind, was mit einer Diagnose von instabiler Angina/Myokardinfarkt ohne ST-Hebung (UA/ NSTEMI) (Patienten mit für eine kardiale Ischämie atypischen Symptomen sollten nicht aufgenommen werden).
Erfüllen Sie mindestens eines der folgenden Kriterien für UA/NSTEMI:
- Alle der folgenden vier Merkmale: *Alter >= bis 65 Jahre; * innerhalb der letzten 7 Tage eingenommenes Aspirin; * zwei oder mehr Angina-Episoden in den letzten 24 Stunden; * drei oder mehr der folgenden Risikofaktoren: Bluthochdruck, hoher Cholesterinspiegel, Familienanamnese, Diabetes, derzeitiger Raucher ODER
- EKG-Veränderungen: Neue oder vermutlich neue ST-Segment-Senkung >=0,1 MV (>=1 mm) oder vorübergehend (=0,1 MV (>=1 mm) in mindestens 2 zusammenhängenden Ableitungen ODER
- Abnormale Herzenzyme innerhalb von 24 Stunden vor der Einschreibung, definiert als erhöhter Troponin I-, T- oder Kreatin-Phosphokinase-MB-Isoenzym (CPK-MB)-Spiegel, der über der Obergrenze des Normalwertes (ULN) OR des Standorts liegt
- Geschichte der koronaren Herzkrankheit mit Dokumentation einer der folgenden: * vorherige Angiographie (Koronarstenose von > 50 %); *vorherige PCI oder Koronararterien-Bypass-Operation (CABG); *vorheriger eindeutiger, dokumentierter Myokardinfarkt.
- Geben Sie eine schriftliche Einverständniserklärung ab, bevor Sie studienbezogene Verfahren einleiten.
Ausschlusskriterien:
- Voraussichtliche Unfähigkeit, eine Angiographie innerhalb von 72 Stunden nach der Randomisierung durchzuführen, und voraussichtliche Unfähigkeit, eine erforderliche Intervention (PCI oder CABG) innerhalb des Index-Krankenhausaufenthalts durchzuführen.
- ST-Streckenhebung von > 1 mm in 2 aufeinanderfolgenden EKG-Ableitungen mit einer Dauer von > 30 Minuten oder neuer Linksschenkelblock oder ein klinisches Syndrom, das mit einem sich akut entwickelnden transmuralen MI vereinbar ist und eine sofortige thrombolytische oder interventionelle Reperfusionstherapie erfordert.
- Kardiogener Schock (systolischer Blutdruck reagiert 30 Minuten lang nicht auf intravenöse Flüssigkeiten oder erfordert intravenöse Druckmittel oder eine intraaortale Ballonpumpe).
- Blutende Diathese oder Vorgeschichte von hämorrhagischem Schlaganfall oder anderer intrazerebraler Blutung, zerebraler arteriovenöser Fehlbildung, zerebralem Aneurysma oder früherem ischämischem Schlaganfall innerhalb der letzten 2 Jahre oder früherem Schlaganfall mit verbleibendem neurologischem Defizit. Gastrointestinale oder urogenitale Blutungen innerhalb der letzten 2 Wochen.
- Thrombozytenzahl
- Patienten unter Warfarin oder Phenprocoumon, es sei denn, sie können sicher abgesetzt werden und die Ausgangs-INR beträgt < das 1,5-fache der Kontrolle.
- Allergie gegen Schweinefleisch oder Schweinefleischprodukte.
- Patienten, die mit 2 oder mehr Dosen niedermolekularen Heparins für die aktuelle Aufnahme vor der Randomisierung begonnen und diese erhalten haben (Patienten, die eine Dosis niedermolekularen Heparins erhalten haben, können weiterhin aufgenommen werden.
- Patienten, die in den 6 Stunden vor der Randomisierung mit Bivalirudin begonnen wurden
- Thrombolytische Therapie oder Anwendung von Abciximab innerhalb der letzten 24 Stunden.
- Behandlung mit einem GPIIb/IIIa-Inhibitor zum Zeitpunkt der Randomisierung, die nicht abgebrochen werden kann.
- Patienten auf Arixtra (Fondaparinux).
Absolute Kontraindikation oder Allergie gegen:
- eines von Enoxaparin, unfraktioniertem Heparin, Bivalirudin oder Aspirin
- sowohl Abciximab als auch Eptifibatid
- sowohl Eptifibatid als auch Tirofiban
- jodhaltiges Kontrastmittel, das nicht prämediziert werden kann
- Kontraindikationen für die Angiographie, einschließlich, aber nicht beschränkt auf schwere periphere Gefäßerkrankungen.
- Angina von sekundären Ursachen.
- Schwangere oder stillende Mütter. Frauen im gebärfähigen Alter müssen vor der Einschreibung einen negativen Urin- oder Serum-Schwangerschaftstest haben.
- Berechnete Serum-Kreatinin-Clearance < 30 ml/min (Ermittelt durch die Cockcroft-Gault-Formel: ((140-Alter in Jahren) x Gewicht in kg)/(814,464 x Kreatinin in mmol/l) oder /(72 x [Kreatinin in mg/dL]). Für Frauen mit 0,85 multiplizieren.
- Vorherige Einschreibung in diese Studie.
- Patienten, die derzeit an einer anderen Prüfpräparatstudie teilnehmen, deren Nachbeobachtungsphase noch nicht abgeschlossen ist.
