Nevrokognitive og nevroatferdsmekanismer for endring etter psykologisk
Alkoholbruksforstyrrelse (AUD) er et betydelig folkehelseproblem, med prevalensrater på 13,9 % for nåværende og 29,1 % for livstidsdiagnose (Grant et al., 2015). AUD skaper skade på individ-, familie- og samfunnsnivå, med en estimert samfunnskostnad på 249 milliarder dollar (Sacks et al., 2015) per år. Forløpet av AUD er typisk preget av perioder med tilbakefall til problematisk drikking (Maisto et al., 2014), noe som signaliserer et behov for bedre behandlinger og forståelse av mekanismer for atferdsendring.
Målet med denne forskningen er å gjennomføre en randomisert klinisk studie med 140 deltakere som har en alkoholbruksforstyrrelse (AUD). Hver deltaker vil fullføre atferdsvurderinger, selvrapporteringsundersøkelser og hjerneavbildning før og etter å ha mottatt psykoterapi for å endre drikkeatferden. Ulike aspekter ved atferdsendring vil bli sett på for å bedre forstå endringer i hjernefunksjon og emosjonell reaktivitet når noen endrer mønstrene for alkoholbruk. De to behandlingene som ble brukt i denne studien har vist seg å være nyttige for å redusere alkoholbruk. Deltakerne vil bli tilfeldig tildelt enten Mindfulness-basert tilbakefallsforebygging (MBRP) eller kognitiv atferdsterapi (CBT) som vil bli gjennomført i løpet av 12 ukentlige terapisesjoner.
Det forventes at det vil være mange endringer i hjernefunksjonen som oppdages når noen reduserer eller slutter med alkoholbruken etter at 12 ukers behandling er fullført.

- Engasjere seg i "farlig og skadelig alkoholbruk" (Babor et al., 2001) basert på en AUDIT-score > 8 for menn og > 7 for kvinner
- Pustepromille på 0,00 ved personlig screening
- Høyrehendt
- Søker eksplisitt hjelp for å drikke
- Alkoholbruk de siste 30 dagene
Ekskluderingskriterier:
- Anamnese med hjerneskade eller nevrologiske diagnoser
- Bevis på nåværende psykose
- Siste års rusavhengighet enn nikotin eller marihuana
- Bevis for nylig bruk av ulovlige stoffer (annet enn marihuana) på en urinskjerm
- Kontraindikasjoner for MR (f.eks. medisinsk utstyr i kroppen)
- Kvinnelige deltakere som tror de kan være gravide, må bestå en uringraviditetsskjerm før hver MR-skanning
- Estimert IQ < 80
- Kan ikke lese eller snakke engelsk flytende
- Historie om alvorlig alkoholabstinens
- For tiden i behandling for alkoholbruk (eller i løpet av de siste 6 månedene)

Aktiv komparator: Kognitiv-atferdsterapi
CBT-tilstanden vil inkludere 8 ukentlige økter på 60 minutter, og vil bli levert i henhold til Epstein & McCrady (2009) manualen for kognitiv-atferdsbehandling, unntatt materiale gitt i plattformbehandlingen.
Behandlingsmanualen og den medfølgende klientarbeidsboken gir detaljerte terapeutinstruksjoner for hver økt, klientøvelser, arbeidsark og lekser.
Behandlingen fokuserer på kognitiv og atferdsmessig mestringstrening, og legger vekt på problemløsning som en overordnet tilnærming til å håndtere drikking.
