GEN3014 sikkerhetsforsøk ved tilbakefall eller refraktær hematologiske maligniteter
En åpen, multisenter, fase 1/2-studie av GEN3014 (HexaBody®-CD38) i residiverende eller refraktært myelomatose og andre hematologiske maligniteter
Legemidlet som skal undersøkes i studien er et antistoff, GEN3014. Siden dette er den første studien av GEN3014 på mennesker, er hovedformålet å evaluere sikkerheten. I tillegg til sikkerhet vil studien bestemme den anbefalte GEN3014-dosen som skal testes i en større gruppe pasienter og vurdere den foreløpige kliniske aktiviteten til GEN3014. GEN3014 vil bli studert ved residiverende eller refraktært myelomatose (også kjent som RRMM) og andre blodkreftformer. Studiet består av 3 deler:
- Doseeskaleringen vil teste økende doser av GEN3014 for å finne et trygt dosenivå som skal testes i de to andre delene.
- Utvidelsesdel A vil videre teste GEN3014-dosen bestemt fra doseeskaleringsdelen.
- Utvidelsesdel B vil sammenligne GEN3014 med daratumumab subkutan (SC), som er et godkjent legemiddel for RRMM.
Forsøkspersonene vil motta GEN3014-undersøkelsesbehandling eller daratumumab; ingen vil få placebo. Studievarigheten vil være ulik for den enkelte deltaker. Samlet sett kan studien pågå inntil 5 år etter siste deltakers første behandling

Detaljert beskrivelse
Denne utprøvingen vil bli utført i 3 deler: Doseeskalering (fase 1), Ekspansjonsdeler, A og B (fase 2).
Alle deltakere i doseeskaleringen vil motta GEN3014, administrert i ulike dosenivåer i 28-dagers sykluser. Dosebegrensende toksisiteter (DLTs) vil bli vurdert i løpet av den første behandlingssyklusen og maksimal tolerert dose (MTD) og/eller anbefalt fase 2-dose (RP2D) vil bli bestemt.
I utvidelsesdel A vil GEN3014 bli ytterligere evaluert og i ytterligere hematologiske maligniteter. I utvidelsesdel B vil GEN3014 bli sammenlignet med daratumumab, head-to-head for å evaluere om GEN3014 kan være mer potent hos anti-CD38 mAb-naive RRMM-pasienter.

Viktige inkluderingskriterier
- Må være minst 18 år.
- Må signere et informert samtykkeskjema (ICF) før alle screeningprosedyrer.
- Må ha ferske benmargsprøver samlet ved Screening.
- Eastern Cooperative Oncology Group (ECOG) ytelsesstatus (PS) score 0, 1 eller 2.
- Har akseptable laboratorietestresultater i løpet av screeningsperioden
- En kvinne med reproduksjonspotensial må godta å bruke adekvat prevensjon under forsøket og i 12 måneder etter siste GEN3014-administrasjon.
- En kvinne i fertil alder må ha et negativt serum beta-humant koriongonadotropin (β-hCG) ved screening.
- En kvinne må samtykke i å ikke donere egg (egg, oocytter) for assistert befruktning under forsøket og i 12 måneder etter å ha mottatt den siste dosen av GEN3014.
En mann som er seksuelt aktiv med en kvinne i fertil alder og ikke har gjennomgått en vasektomi, må godta å bruke en barrieremetode for prevensjon.
Spesifikt for RRMM:
Må ha dokumentert myelomatose som definert av kriteriene nedenfor og ha bevis for sykdomsprogresjon på det siste tidligere behandlingsregimet basert på IMWG-kriterier:
Forhåndsdokumentasjon av monoklonale plasmaceller i benmargen
≥10 % eller tilstedeværelse av et biopsi-påvist plasmacytom. og
- Målbar sykdom ved baseline som definert av ett av følgende:
- IgG, IgA, IgD eller IgM myelom: Serum M-proteinnivå ≥0,5 g/dL (≥5 g/L) eller urin M-proteinnivå ≥200 mg/24 timer; Eller
- Lett kjede myelom: Serum Ig fri lett kjede (FLC) ≥10 mg/dL og unormalt serum Ig kappa lambda FLC ratio Merk: Personer med RRMM må ha uttømt standardbehandlinger, etter utrederens skjønn.
- For anti-CD38 mAb-naiv RRMM-kohort: Personen har mottatt minst 3 tidligere behandlingslinjer inkludert en PI og en IMiD i hvilken som helst rekkefølge, eller er dobbelt refraktær overfor en PI og en IMiD; eller personen mottok ≥ 2 tidligere behandlingslinjer hvis 1 av disse linjene inkluderte en kombinasjon av PI og IMiD. Merk: Forsøkspersonene skal ikke ha mottatt anti-CD38-antistoff. Anti-CD38 mAb-naive RRMM-personer vil bli rekruttert fra land der anti-CD38-terapier ikke er tilgjengelige.
For anti-CD38 mAb-behandlet RRMM-kohort: Personen har mottatt minst 2 tidligere behandlingslinjer og må ha seponert daratumumab eller isatuximab i minst 4 uker før den første dosen av GEN3014. Merk: Forsøkspersonene skal ikke ha mottatt noe annet anti-CD38-antistoff bortsett fra daratumumab eller isatuximab.
Spesifikt for AML:
- Tilbakefallende eller refraktær AML, både de novo eller sekundær; må ha mislyktes i all konvensjonell terapi. Akutt promyelocytisk leukemi (APL) er ekskludert fra denne studien. Merk: Tilbakefall er definert av BM-blaster ≥5 % hos pasienter som har vært i CR tidligere, eller gjenopptreden av blaster i blodet, eller utvikling av ekstramedullær AML. Refraktær er definert som å ikke kunne oppnå en CR etter den første behandlingen.
- Person med residiverende AML som har mottatt minst 2 tidligere behandlinger for AML med unntak av hydroksyurea.
- • Person med refraktær AML som har mottatt minst 1 tidligere behandlingslinje for AML med unntak av hydroksyurea.
Forsøkspersonens forventede levealder ved screening vurderes til å være minst 3 måneder.
Spesifikt for DLBCL
- Tilbakefallende eller refraktær DLBCL, både de novo eller histologisk transformert. Merk: Tilbakefallende sykdom er definert som gjenopptreden eller vekst av lymfom etter minst 6 måneders varighet av respons. Refraktær sykdom er definert som manglende respons etter minst 2 behandlingssykluser eller gjenopptreden etter en varighet av respons på
- Mottatt minst 2 tidligere linjer med systemisk terapi, hvorav 1 var en CD20-holdig kjemoimmunterapi.
Ha minst 1 målbart sykdomssted:
- En fluorodeoksyglukose (FDG)-positronemisjonstomografi (PET) computertomografi (CT) skanning som viser positiv lesjon forenlig med CT (eller magnetisk resonanstomografi [MRI])-definerte anatomiske tumorsteder og
- En CT-skanning (eller MR) med involvering av ≥2 klart avgrensede lesjoner/noder med lang akse >1,5 cm og kort akse >1,0 cm; eller 1 tydelig avgrenset lesjon/node med lang akse >2,0 cm og kort akse ≥1,0 cm.
- Må ha tilgjengelig arkiv eller ferskt svulstvev eller begge deler for å sende inn til et sentrallaboratorium for CD38-analyse
Nøkkeleksklusjonskriterier
- Tidligere behandling med et anti-CD38-antistoff unntatt daratumumab eller isatuximab.
- Behandling med et anti-kreftmiddel, kjemoterapi, strålebehandling eller større kirurgi innen 2 uker før første dose av studiebehandlingen.
- Behandling med et forsøkslegemiddel innen 4 uker eller 5 halveringstider, avhengig av hva som er kortest, før den første dosen av studiebehandlingen.
- Kumulativ dose av kortikosteroider mer enn tilsvarende ≥140 mg prednison innen 2 uker før den første dosen av studiebehandlingen.
- Har klinisk signifikant hjertesykdom.
- Toksisiteter fra tidligere anti-kreftbehandlinger har ikke gått over til baseline-nivåer eller til grad 1 eller mindre, bortsett fra alopecia og perifer nevropati.
- Primær svulst i sentralnervesystemet (CNS) eller kjent CNS-involvering ved screening.
- Har kjent historie/positiv serologi for hepatitt B
- Kjent sykehistorie eller pågående hepatitt C-infeksjon som ikke er kurert.
- HIV-positiv ved screening
- Mottar for øyeblikket andre undersøkelsesmidler.
- En kvinne som er gravid eller ammer, eller som planlegger å bli gravid mens hun deltar i denne studien eller innen 12 måneder etter siste dose av studiebehandlingen.
En mann som planlegger å bli far til et barn mens han er registrert i denne studien eller innen 12 måneder etter siste dose av studiebehandlingen.

- • Aktiv graft-versus-host-sykdom som krever immunsuppressiv behandling. All immunsuppressiv medisin (f.eks. kalsineurinhemmere) må stoppes ≥4 uker før den første dosen av GEN3014.

DLTs were defined as: All Grade 5 toxicities, Grade 4 thrombocytopenia, neutropenia or anemia and Grade 3/4 febrile neutropenia and hemorrhage associated with thrombocytopenia, all non-hematological toxicities of grade ≥3 (with exceptions per protocol), Grade 4 tumor lysis syndrome (TLS), Grade 4 infusion-related reaction (IRR), and any liver toxicity of elevated alanine aminotransferase (ALT) or aspartate aminotransferase (AST) ≥3 times or greater above the upper limit of normal (ULN) with serum total bilirubin of ≥2 times the upper limit of normal, without findings of cholestasis and in the absence of alternative etiologies.
DLTs were graded for severity according to National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE) v5.0.

An AE was any unfavorable and unintended sign (including an abnormal laboratory finding), symptom, or disease (new or exacerbated) temporally associated with the use of a medicinal product.
An SAE was defined as an AE that met 1 of the following criteria: fatal or life-threatening, resulted in persistent or significant disability/incapacity, a congenital anomaly/birth defect, was medically significant (jeopardized the participant or may have required medical or surgical intervention to prevent one of the outcomes listed above), required inpatient hospitalization or prolongation of existing hospitalization.

Expansion Part A: Objective Response Rate (ORR)

ORR was defined as the percentage of participants with a partial response (PR) or better based on International Myeloma Working Group (IMWG) criteria for RRMM participants and based on Lugano criteria for DLBCL participants.
Per IMWG criteria, PR was defined as ≥50% reduction of serum M-protein plus reduction in 24-h urinary M-protein by ≥90% or to <200 mg per 24 h.
In addition to these criteria, if present at baseline, a ≥50% reduction in the size (sum of the product of the diameters [SPD]) of soft tissue plasmacytomas was also required.
Per Lugano criteria, PR was defined as ≥50% decrease in SPD of up to 6 target measurable nodes and extranodal sites, absent/normal, regressed, but no increase in non-measurable lesions, spleen regressed >50% in length beyond normal (13 centimeters [cm]), and no new lesions.

Dose Escalation and Expansion Part A: Clinical Benefit Rate (CBR)

CBR was defined as the percentage of participants with a best overall response of minimal response (MR), partial response (PR), very good partial response (VGPR), complete response (CR) or stringent complete response (sCR) as determined by the investigator per IMWG response criteria for RRMM participants.
MR=≥25% but ≤49% reduction of serum M-protein, reduction of 24-h urine by 50%-89% and if present at baseline, ≥50% reduction in the size (SPD) of soft tissue plasmacytomas.
PR=≥50% reduction of serum M-protein, reduction in 24-h urinary M-protein by ≥90% or to <200mg/24h and if present at baseline,≥50% reduction in SPD of soft tissue plasmacytomas.
VGPR=Serum+urine M-protein detectable by IFE but not electrophoresis or ≥90% reduction in serum M-protein+urine M-protein level <100mg/24h.
CR=Negative IFE on serum/urine + disappearance of soft tissue plasmacytomas and ≤5% plasma cells in bone marrow aspirates.
sCR=CR+normal FLC ratio, absence of clonal cells in bone marrow biopsy by IHC.

An AE was any untoward medical occurrence in a participant or clinical trial participant, temporally associated with the use of a medicinal product, whether or not considered related to the medicinal product.
An AE was any unfavorable and unintended sign (including an abnormal laboratory finding), symptom, or disease (new or exacerbated) temporally associated with the use of a medicinal product.
An SAE was defined as an AE that met 1 of the following criteria: fatal or life-threatening, resulted in persistent or significant disability/incapacity, a congenital anomaly/birth defect, was medically significant (jeopardized the participant or may have required medical or surgical intervention to prevent one of the outcomes listed above), required inpatient hospitalization or prolongation of existing hospitalization.

Expansion Part B: Percentage of Participants With Very Good Partial Response (VGPR) or Better

Based on Investigator Assessment per IMWG 2016 Criteria, VGPR was defined as serum and urine M-protein detectable by immunofixation (IFE) but not on electrophoresis or ≥90% reduction in serum M-protein plus urine M-protein level <100 mg per 24 h.
Data are reported for the percentage of participants with a VGPR or better.

Expansion Part B: Percentage of Participants With Complete Response (CR) or Better

CR was defined as negative IFE on the serum and urine and disappearance of any soft tissue plasmacytomas and ≤5% plasma cells in bone marrow aspirates based on Investigator assessment per IMWG response criteria.
Data are reported for the percentage of participants with CR or better.

Expansion Part B: Time to Next Therapy (TTNT)

TTNT for participants in the Expansion Part B was defined as the time from randomization to the start of subsequent anti-cancer therapy.
Participants who withdrew consent or were lost to follow up or died due to causes other than disease progression were censored at their last disease assessment.

Expansion Part B: Number of Participants With Anti-GEN3014 Antibodies and Anti-Daratumumab Antibodies
Tidsramme: Cycle 1 Day 22 up to a maximum of approximately 23.9 months (cycles = 28 days)

Venous blood samples were drawn for analysis of ADAs to GEN3014 and daratumumab.
In the GEN3014 arm, participants were tested for anti-GEN3014-antibodies; in the daratumumab arm, participants were tested for anti-daratumumab-antibodies.
