Un essai ouvert, multicentrique, de phase 1/2 de GEN3014 (HexaBody®-CD38) dans le myélome multiple récidivant ou réfractaire et d'autres hémopathies malignes

Le médicament qui sera étudié dans l'étude est un anticorps, GEN3014. Comme il s'agit de la première étude de GEN3014 chez l'homme, l'objectif principal est d'évaluer la sécurité. Outre la sécurité, l'étude déterminera la dose recommandée de GEN3014 à tester dans un groupe plus large de patients et évaluera l'activité clinique préliminaire de GEN3014. GEN3014 sera étudié dans le myélome multiple récidivant ou réfractaire (également connu sous le nom de RRMM) et d'autres cancers du sang. L'étude se compose de 3 parties :

  1. L'escalade de dose testera des doses croissantes de GEN3014 pour trouver un niveau de dose sûr à tester dans les deux autres parties.
  2. La partie d'extension A testera davantage la dose GEN3014 déterminée à partir de la partie d'escalade de dose.
  3. La partie B de l'extension comparera GEN3014 au daratumumab sous-cutané (SC), qui est un médicament approuvé pour la RRMM.

Les sujets recevront un traitement expérimental GEN3014 ou du daratumumab ; aucun ne recevra un placebo. La durée de l'étude sera différente pour les participants individuels. Dans l'ensemble, l'étude peut se poursuivre jusqu'à 5 ans après le premier traitement du dernier participant

Aperçu de l'étude

Description détaillée

Cet essai se déroulera en 3 parties : Dose Escalation (phase 1), Expansion Parts, A et B (phase 2).

Tous les participants à l'escalade de dose recevront GEN3014, administré à différents niveaux de dose dans des cycles de 28 jours. Les toxicités limitant la dose (DLT) seront évaluées au cours du premier cycle de traitement et la dose maximale tolérée (MTD) et/ou la dose recommandée de phase 2 (RP2D) seront déterminées.

Dans la partie A de l'expansion, GEN3014 sera évalué plus en détail et dans d'autres hémopathies malignes. Dans la partie B de l'extension, GEN3014 sera comparé au daratumumab, face à face, pour évaluer si GEN3014 peut être plus puissant chez les patients RRMM naïfs d'AcM anti-CD38.

Critères d'inclusion clés

  1. Doit être âgé d'au moins 18 ans.
  2. Doit signer un formulaire de consentement éclairé (ICF) avant toute procédure de dépistage.
  3. Doit avoir des échantillons de moelle osseuse frais prélevés lors du dépistage.
  4. Score de l'état de performance (EP) de l'Eastern Cooperative Oncology Group (ECOG) de 0, 1 ou 2.
  5. A des résultats de test de laboratoire acceptables pendant la période de sélection
  6. Une femme en âge de procréer doit accepter d'utiliser une contraception adéquate pendant l'essai et pendant 12 mois après la dernière administration de GEN3014.
  7. Une femme en âge de procréer doit avoir une bêta-gonadotrophine chorionique humaine (β-hCG) sérique négative lors du dépistage.
  8. Une femme doit accepter de ne pas donner d'ovules (ovules, ovocytes) pour la procréation assistée pendant l'essai et pendant 12 mois après avoir reçu la dernière dose de GEN3014.
  9. Un homme qui est sexuellement actif avec une femme en âge de procréer et qui n'a pas subi de vasectomie doit accepter d'utiliser une méthode contraceptive de barrière.

    Spécifique pour RRMM :

  10. Doit avoir documenté un myélome multiple tel que défini par les critères ci-dessous et avoir des preuves de progression de la maladie sur le schéma de traitement antérieur le plus récent basé sur les critères IMWG :

    • Documentation préalable des plasmocytes monoclonaux dans la moelle osseuse

      ≥ 10 % ou présence d'un plasmocytome prouvé par biopsie. et

    • Maladie mesurable au départ telle que définie par l'un des éléments suivants :
    • Myélome IgG, IgA, IgD ou IgM : taux sérique de protéine M ≥ 0,5 g/dL (≥ 5 g/L) ou taux urinaire de protéine M ≥ 200 mg/24 heures ; Ou
    • Myélome à chaîne légère : chaîne légère libre (FLC) d'Ig sérique ≥ 10 mg/dL et rapport FLC d'Ig kappa lambda anormal
  11. Pour la cohorte RRMM naïve d'AcM anti-CD38 : le sujet a reçu au moins 3 lignes de traitement antérieures, y compris un PI et un IMiD dans n'importe quel ordre, ou est double réfractaire à un PI et à un IMiD ; ou le sujet a reçu ≥ 2 lignes de traitement antérieures si 1 de ces lignes comprenait une combinaison d'IP et d'IMiD. Remarque : Les sujets ne doivent avoir reçu aucun anticorps anti-CD38. Les sujets RRMM naïfs d'AcM anti-CD38 seront recrutés dans des pays où les thérapies anti-CD38 ne sont pas disponibles.
  12. Pour la cohorte RRMM traitée par mAb anti-CD38 : le sujet a reçu au moins 2 lignes de traitement antérieures et doit avoir arrêté le daratumumab ou l'isatuximab pendant au moins 4 semaines avant la première dose de GEN3014. Remarque : les sujets ne doivent avoir reçu aucun autre anticorps anti-CD38, à l'exception du daratumumab ou de l'isatuximab.

    Spécifique pour la LMA :

  13. LAM récidivante ou réfractaire, de novo ou secondaire ; doit avoir échoué à tous les traitements conventionnels. La leucémie aiguë promyélocytaire (LPA) est exclue de cet essai. Remarque : La rechute est définie par des blastes de BM ≥ 5 % chez les patients qui ont déjà été en RC, ou la réapparition de blastes dans le sang, ou le développement d'AML extramédullaire. Réfractaire est défini comme étant incapable d'obtenir une RC après le traitement initial.
  14. Sujet atteint de LAM en rechute ayant reçu au moins 2 traitements antérieurs pour la LAM à l'exception de l'hydroxyurée.
  15. • Sujet atteint de LAM réfractaire ayant reçu au moins 1 ligne de traitement antérieure pour la LAM à l'exception de l'hydroxyurée.
  16. L'espérance de vie du sujet lors du dépistage est estimée à au moins 3 mois.

    Spécifique pour DLBCL

  17. DLBCL récidivant ou réfractaire, à la fois de novo ou histologiquement transformé. Remarque : La maladie récidivante est définie comme la réapparition ou la croissance d'un lymphome après au moins 6 mois de réponse. La maladie réfractaire est définie comme une absence de réponse après au moins 2 cycles de traitement ou une réapparition après une durée de réponse de
  18. A reçu au moins 2 lignes antérieures de thérapie systémique, dont 1 étant une chimio-immunothérapie contenant du CD20.
  19. Avoir au moins 1 site mesurable de la maladie :

    • Une tomodensitométrie (TDM) au fluorodésoxyglucose (FDG)-tomographie par émission de positrons (TEP) montrant une lésion positive compatible avec les sites tumoraux anatomiques définis par TDM (ou imagerie par résonance magnétique [IRM]) Et
    • Une tomodensitométrie (ou IRM) avec atteinte de ≥ 2 lésions/nœuds clairement délimités avec un axe long > 1,5 cm et un axe court > 1,0 cm ; ou 1 lésion/nœud clairement délimité avec un axe long > 2,0 cm et un axe court ≥ 1,0 cm.
  20. Doit avoir des tissus tumoraux d'archives ou frais disponibles ou les deux à soumettre à un laboratoire central pour le test CD38

Principaux critères d'exclusion

  1. Traitement antérieur par un anticorps anti-CD38 sauf daratumumab ou isatuximab.
  2. Traitement avec un agent anticancéreux, chimiothérapie, radiothérapie ou chirurgie majeure dans les 2 semaines précédant la première dose du traitement à l'étude.
  3. Traitement avec un médicament expérimental dans les 4 semaines ou 5 demi-vies, selon la plus courte des deux, avant la première dose du traitement à l'étude.
  4. Dose cumulée de corticostéroïdes supérieure à l'équivalent de ≥ 140 mg de prednisone dans les 2 semaines précédant la première dose du traitement à l'étude.
  5. A une maladie cardiaque cliniquement significative.
  6. Les toxicités des thérapies anticancéreuses précédentes ne se sont pas résolues aux niveaux de base ou au grade 1 ou moins, à l'exception de l'alopécie et de la neuropathie périphérique.
  7. Tumeur primaire du système nerveux central (SNC) ou atteinte connue du SNC lors du dépistage.
  8. A des antécédents connus / une sérologie positive pour l'hépatite B
  9. Antécédents médicaux connus ou infection à l'hépatite C en cours qui n'a pas été guérie.
  10. Séropositif au dépistage
  11. Reçoit actuellement tout autre agent expérimental.
  12. Une femme enceinte ou qui allaite, ou qui envisage de tomber enceinte pendant son inscription à cet essai ou dans les 12 mois suivant la dernière dose du traitement à l'étude.

Per IMWG criteria, PR was defined as ≥50% reduction of serum M-protein plus reduction in 24-h urinary M-protein by ≥90% or to <200 mg per 24 h. In addition to these criteria, if present at baseline, a ≥50% reduction in the size (sum of the product of the diameters [SPD]) of soft tissue plasmacytomas was also required. Per Lugano criteria, PR was defined as ≥50% decrease in SPD of up to 6 target measurable nodes and extranodal sites, absent/normal, regressed, but no increase in non-measurable lesions, spleen regressed >50% in length beyond normal (13 centimeters [cm]), and no new lesions.

ORR was defined as the percentage of participants with a PR or better. The response in RRMM Cohorts was assessed following IMWG criteria 2016. Per IMWG criteria, PR was defined as ≥50% reduction of serum M-protein plus reduction in 24-h urinary M-protein by ≥90% or to <200 mg per 24 h. In addition to these criteria, if present at baseline, a ≥50% reduction in the size (SPD) of soft tissue plasmacytomas was also required.

Dose Escalation and Expansion Part A: Clinical Benefit Rate (CBR)

CBR was defined as the percentage of participants with a best overall response of minimal response (MR), partial response (PR), very good partial response (VGPR), complete response (CR) or stringent complete response (sCR) as determined by the investigator per IMWG response criteria for RRMM participants. MR=≥25% but ≤49% reduction of serum M-protein, reduction of 24-h urine by 50%-89% and if present at baseline, ≥50% reduction in the size (SPD) of soft tissue plasmacytomas. PR=≥50% reduction of serum M-protein, reduction in 24-h urinary M-protein by ≥90% or to <200mg/24h and if present at baseline,≥50% reduction in SPD of soft tissue plasmacytomas. VGPR=Serum+urine M-protein detectable by IFE but not electrophoresis or ≥90% reduction in serum M-protein+urine M-protein level <100mg/24h. CR=Negative IFE on serum/urine + disappearance of soft tissue plasmacytomas and ≤5% plasma cells in bone marrow aspirates. sCR=CR+normal FLC ratio, absence of clonal cells in bone marrow biopsy by IHC.

An AE was any unfavorable and unintended sign (including an abnormal laboratory finding), symptom, or disease (new or exacerbated) temporally associated with the use of a medicinal product. An SAE was defined as an AE that met 1 of the following criteria: fatal or life-threatening, resulted in persistent or significant disability/incapacity, a congenital anomaly/birth defect, was medically significant (jeopardized the participant or may have required medical or surgical intervention to prevent one of the outcomes listed above), required inpatient hospitalization or prolongation of existing hospitalization. Expansion Part B: Percentage of Participants With Very Good Partial Response (VGPR) or Better

Based on Investigator Assessment per IMWG 2016 Criteria, VGPR was defined as serum and urine M-protein detectable by immunofixation (IFE) but not on electrophoresis or ≥90% reduction in serum M-protein plus urine M-protein level <100 mg per 24 h. Data are reported for the percentage of participants with a VGPR or better.

Expansion Part B: Percentage of Participants With Complete Response (CR) or Better

CR was defined as negative IFE on the serum and urine and disappearance of any soft tissue plasmacytomas and ≤5% plasma cells in bone marrow aspirates based on Investigator assessment per IMWG response criteria. Data are reported for the percentage of participants with CR or better.

Expansion Part B: Time to Next Therapy (TTNT)

TTNT for participants in the Expansion Part B was defined as the time from randomization to the start of subsequent anti-cancer therapy. Participants who withdrew consent or were lost to follow up or died due to causes other than disease progression were censored at their last disease assessment.

Expansion Part B: Number of Participants With Anti-GEN3014 Antibodies and Anti-Daratumumab Antibodies

Venous blood samples were drawn for analysis of ADAs to GEN3014 and daratumumab. In the GEN3014 arm, participants were tested for anti-GEN3014-antibodies; in the daratumumab arm, participants were tested for anti-daratumumab-antibodies.
