Een open-label, multicenter, fase 1/2-onderzoek van GEN3014 (HexaBody®-CD38) bij recidiverend of refractair multipel myeloom en andere hematologische maligniteiten

Het medicijn dat in de studie onderzocht gaat worden is een antilichaam, GEN3014. Aangezien dit de eerste studie van GEN3014 bij mensen is, is het belangrijkste doel om de veiligheid te evalueren. Naast de veiligheid zal de studie de aanbevolen GEN3014-dosis bepalen die moet worden getest in een grotere groep patiënten en de voorlopige klinische activiteit van GEN3014 beoordelen. GEN3014 zal worden bestudeerd bij gerecidiveerd of refractair multipel myeloom (ook bekend als RRMM) en andere bloedkankers. Het onderzoek bestaat uit 3 delen:

  1. De dosisescalatie zal toenemende doses van GEN3014 testen om een ​​veilig dosisniveau te vinden dat in de andere twee delen kan worden getest.

Gedetailleerde beschrijving

Deze proef zal worden uitgevoerd in 3 delen: dosisescalatie (fase 1), uitbreidingsdelen, A en B (fase 2).

Alle deelnemers aan de dosisescalatie zullen GEN3014 ontvangen, toegediend in verschillende dosisniveaus in cycli van 28 dagen. Dosisbeperkende toxiciteiten (DLT's) zullen tijdens de eerste behandelingscyclus worden beoordeeld en de maximaal getolereerde dosis (MTD) en/of de aanbevolen fase 2-dosis (RP2D) zullen worden bepaald.

In uitbreidingsdeel A zal GEN3014 verder worden geëvalueerd en bij aanvullende hematologische maligniteiten. In Expansion Part B zal GEN3014 worden vergeleken met daratumumab, head-to-head om te evalueren of GEN3014 mogelijk krachtiger is bij anti-CD38 mAb-naïeve RRMM-patiënten.

Belangrijkste opnamecriteria

  1. Moet minimaal 18 jaar oud zijn.
  2. Moet een geïnformeerd toestemmingsformulier (ICF) ondertekenen voorafgaand aan screeningprocedures.
  3. Moet verse beenmergmonsters laten verzamelen bij screening.
  4. Eastern Cooperative Oncology Group (ECOG) performance status (PS) score 0, 1 of 2.
  5. Heeft aanvaardbare laboratoriumtestresultaten tijdens de screeningperiode
  6. Een vruchtbare vrouw moet ermee instemmen om adequate anticonceptie te gebruiken tijdens het onderzoek en gedurende 12 maanden na de laatste toediening van GEN3014.
  7. Een vrouw in de vruchtbare leeftijd moet bij de screening een negatief serum bèta-humaan choriongonadotrofine (β-hCG) hebben.

    Specifiek voor RRMM:

  10. Moet multipel myeloom hebben gedocumenteerd zoals gedefinieerd door de onderstaande criteria en bewijs hebben van ziekteprogressie op het meest recente eerdere behandelingsregime op basis van IMWG-criteria:

    • Voorafgaande documentatie van monoklonale plasmacellen in het beenmerg

      ≥10% of aanwezigheid van een door biopsie bewezen plasmacytoom. en

    • Meetbare ziekte bij baseline zoals gedefinieerd door een van de volgende:

    Specifiek voor AML:

  13. Recidiverende of refractaire AML, zowel de novo als secundair; moet alle conventionele therapie hebben gefaald. Acute promyelocytische leukemie (APL) is uitgesloten van deze studie. Opmerking: Terugval wordt gedefinieerd door BM-blasten ≥5% bij patiënten die eerder CR hebben gehad, of opnieuw verschijnen van blasten in het bloed, of ontwikkeling van extramedullaire AML. Refractair wordt gedefinieerd als het niet kunnen bereiken van een CR na de initiële therapie.

Venous blood samples were collected for analyzing concentrations of GEN3014. For the RRMM Dose Escalation 0.2/0.6 mg/kg arm, the participant underwent intraparticipant dose escalation in Cycle 1, where the participant received 0.2 mg/kg during the first week and 0.6 mg/kg from second week and beyond. Therefore, PK parameters were additionally calculated for Cycle 1 Day 8 dosing only in the RRMM Dose Escalation 0.2/0.6 mg/kg arm to account for the change in dosing over Cycle 1 in this arm.

In addition to these criteria, if present at baseline, a ≥50% reduction in the size (SPD) of soft tissue plasmacytomas was also required.

MR=≥25% but ≤49% reduction of serum M-protein, reduction of 24-h urine by 50%-89% and if present at baseline, ≥50% reduction in the size (SPD) of soft tissue plasmacytomas. PR=≥50% reduction of serum M-protein, reduction in 24-h urinary M-protein by ≥90% or to <200mg/24h and if present at baseline,≥50% reduction in SPD of soft tissue plasmacytomas. VGPR=Serum+urine M-protein detectable by IFE but not electrophoresis or ≥90% reduction in serum M-protein+urine M-protein level <100mg/24h. CR=Negative IFE on serum/urine + disappearance of soft tissue plasmacytomas and ≤5% plasma cells in bone marrow aspirates. sCR=CR+normal FLC ratio, absence of clonal cells in bone marrow biopsy by IHC.
