Kryteria przyjęcia:
- Wiek ≥ 18 lat
- Podejrzenie glejaka (stopień II, III lub IV)
- Przedoperacyjna perfuzja MR (wzmacnianie guzów)
- Przedoperacyjna spektroskopia MR 2-HG (guzy niewzmacniające się) Pacjent wskazany do resekcji chirurgicznej, standardowej radioterapii i standardowej chemioterapii jako standard opieki
- Stan sprawności Karnofsky'ego ≥ 60
- Oczekiwana długość życia > 12 tygodni
- Zdolność do przestrzegania procedur badania i obserwacji
Kryteria wyłączenia:
- Wcześniejsze rozpoznanie glejaka wewnątrzczaszkowego
- Inny nowotwór wymagający leczenia systemowego w ciągu 3 lat
- Niemożność dostarczenia 6000 szarości promieniowania do mózgu
- Konieczność pilnej interwencji paliatywnej w przypadku choroby podstawowej (np. zbliżającej się przepukliny)
- Dowody skazy krwotocznej lub koagulopatii
- Historia ropnia śródmózgowego w ciągu 6 miesięcy przed Dniem 0
- Poważny zabieg chirurgiczny, otwarta biopsja lub znaczny uraz urazowy w ciągu 28 dni przed Dniem 0, przewidywanie konieczności przeprowadzenia poważnego zabiegu chirurgicznego w trakcie badania
- Kobiety w ciąży
- Osoby, które nie mogą poddać się badaniu MRI z kontrastem

Eksperymentalny: Kohorta 1, grupa B (niewzmacniający się guz)
Pacjenci z grupy B zostaną poddani standardowemu przedoperacyjnemu obrazowaniu guza i skanowi spektroskopii 2HG przed resekcją chirurgiczną.
Pacjenci z grupy A zostaną poddani standardowemu obrazowaniu śródoperacyjnemu wraz ze skanem spektroskopii 2HG podczas resekcji chirurgicznej.

Eksperymentalny: Kohorta 2
Podczas analizy dla kohorty 1 stwierdziliśmy istotne różnice w próbkach przed MRI i po śródoperacyjnym MRI. Utworzono kohortę 2, która obejmie 10 dodatkowych pacjentów, aby potwierdzić te różnice. W przeciwieństwie do kohorty 1, do wykonania tego porównania nie jest potrzebne zaawansowane obrazowanie (spektroskopia 2-HG i perfuzja MR). The standard of care intraoperative MRI sequences used routinely will be sufficient for these 10 new samples.
