Den sexuella hälsan hos patienter med ändtarmscancer: En pilotstudie
Vi vet att behandling av rektal-, kolorektal- eller analcancer kan påverka sexuell funktion. Denna studie syftar till att lära sig:
- Hur behandlingen påverkar det känslomässiga och sociala välbefinnandet.
- Hur ett nytt utbildningsprogram för sexuell hälsa påverkar dig.
Cancerpatienter söker utbildning av många skäl. Det hjälper dem ofta att klara sig eftersom de kan uttrycka sina känslor med en utbildad professionell. Denna studie är öppen för personer som har behandlats för rektal-, kolorektal- eller analcancer. Det innebär en ny typ av rådgivningsprogram för sexuell hälsa. Det kommer att lära deltagarna färdigheter som de kan använda för att förbättra sina sexliv. Dessa färdigheter kan också förbättra fysiskt och känslomässigt välbefinnande. Vi kommer att jämföra den nya typen av sexuella hälsoprogram med den standardvård som patienter får efter behandling för rektal-, kolorektal- eller analcancer.

Detta är en tvåårig randomiserad klinisk prövning som undersöker genomförbarheten, tolerabiliteten, acceptansen (inklusive följsamhet) och effektiviteten av en intervention utformad för att förbättra den sexuella funktionen hos överlevande från rektalcancer.
Vi har slutfört inskrivningen av kvinnliga patienter och håller för närvarande på att samla in uppföljningsdata. Rekrytering av manliga patienter pågår. Manliga deltagare kommer nu att randomiseras i ett 3:1-förhållande till antingen interventionsvillkoret (n = 40) eller kontrollvillkoret (n = 40).

Inklusionskriterier:
- Minst sex månader efter strålning och/eller operation för stadium I-III rektalt adenokarcinom eller rectosigmoid för cancer med anastomos vid 15 cm eller lägre och avslutad behandling
- Minst sex månader efter strålning och/eller kemoterapi för analcancer.
- Har inga tecken på sjukdom eller återfall
- För kvinnor indikerar Ange måttlig tillfredsställelse eller lägre på deras övergripande sexuella liv (en poäng på "4" eller lägre på frågan "Under de senaste 4 veckorna, hur nöjd har du varit med ditt övergripande sexuella liv?"
- För män ange, och en poäng på "3" eller lägre på frågan "Hur skulle du bedöma ditt självförtroende för att du skulle kunna få och behålla en erektion" för män) och en poäng på "2" eller högre på frågan "Hur mycket stör detta problem dig?
- Enligt de samtyckande professionella interventionisterna kan de kommunicera på engelska tillräckligt bra för att arbeta med engelsktalande och slutföra studiebedömningarna.
- Ålder 21 år eller äldre.
Exklusions kriterier:
- Betydande kognitiv eller psykiatrisk störning tillräcklig, enligt utredarens bedömning, för att utesluta deltagande i en psykoterapeutisk intervention.
- Deltar aktivt i protokoll 06-151
- Deltagit i fokusgrupp eller kvalitativ intervju.

vanlig vård (UC) som är den standardvård som patienterna får

Deltagare som randomiserats till Usual Care-armen kommer att få standardvård.
För kvinnor definieras detta som den vård som deltagare som inte är med i insatsen får; vi kommer att fråga dessa deltagare vilken psykosocial vård de får för att spåra vanlig vård.
För män kommer "vanlig vård" att bestå av en standardiserad remiss till mäns Sexual Medicine Program vid MSKCC och skriftlig information i form av American Cancer Society (ACS) häfte om Sexuality after Cancer.
Individer som randomiserats till Usual Care kommer inte att få några ytterligare rådgivningssessioner från studieinterventionisterna förrän de är klara med uppföljningsintervjuerna och sedan kommer det att erbjudas dem.
Uppföljande frågeformulär kommer att utvärdera alla studiedeltagare 4 och 8 månader efter baslinjen.

Canceröverlevnadsintervention-sexuell hälsa (CSI-SH) plus vanlig vård (USA)
|
Detta innebär fyra individuella sessioner på 1 timme med ytterligare tre telefonbooster-/recensionssessioner mellan de fyra sessionerna.
Sessioner kan genomföras personligen eller genomföras via telefon.
Även om alla ansträngningar kommer att göras för att slutföra boostersessionen mellan rådgivningssessionerna, kan de hoppas över om patienten inte är tillgänglig för att slutföra boostersamtalen.
Sessionerna ett och två kommer att fokusera på rehabiliteringstekniker och verktyg kommer att tillhandahållas och diskuteras med deltagarna under dessa första två sessioner.

Scores of 0 to 5 are awarded to each of the 15 questions.
The five domains assessed include Erectile Function (Max score 30), Orgasmic Function (Max score 10), Sexual Desire (Max score 10), Intercourse Satisfaction (Max score 15) and Overall Satisfaction (Max score 10).
Overall Scoring: 1-10: Severe Erectile Dysfunction, 11-16: Moderate dysfunction,17-21: Mild to moderate dysfunction, 22-25: Mild dysfunction, 26-30: No dysfunction.

It is composed of 30-items, which factor into scales that evaluate physical functioning and role functioning, as well as emotional, social, and cognitive functioning and global QOL.
Three symptom scales measure fatigue, pain, and emesis, while six single items assess financial impact, dyspnea, sleep disturbance, appetite, diarrhea, and constipation.
