De seksuele gezondheid van rectumkankerpatiënten: een pilotstudie
We weten dat de behandeling van rectale, colorectale of anale kanker invloed kan hebben op de seksuele functie. Deze studie heeft tot doel te leren:
- Hoe de behandeling het emotionele en sociale welzijn beïnvloedt.
- Hoe een nieuw voorlichtingsprogramma over seksuele gezondheid u beïnvloedt.
Kankerpatiënten zoeken om tal van redenen onderwijs. Het helpt hen vaak om ermee om te gaan, omdat ze hun gevoelens kunnen uiten bij een getrainde professional. Deze studie staat open voor personen die zijn behandeld voor rectale, colorectale of anale kanker. Het gaat om een nieuw soort counselingprogramma voor seksuele gezondheid. Het leert deelnemers vaardigheden die ze kunnen gebruiken om hun seksleven te verbeteren. Deze vaardigheden kunnen ook het fysieke en emotionele welzijn verbeteren. We vergelijken het nieuwe type programma voor seksuele gezondheid met de standaardzorg die patiënten krijgen na een behandeling voor endeldarm-, darm- of anuskanker.

Dit is een gerandomiseerde klinische studie van twee jaar waarin de haalbaarheid, verdraagbaarheid, aanvaardbaarheid (inclusief therapietrouw) en werkzaamheid worden onderzocht van een interventie die is ontworpen om het seksueel functioneren van overlevenden van endeldarmkanker te verbeteren.
We hebben de inschrijving van vrouwelijke patiënten voltooid en verzamelen momenteel follow-upgegevens. Werving voor mannelijke patiënten is aan de gang. Mannelijke deelnemers worden nu gerandomiseerd in een verhouding van 3:1 naar de interventieconditie (n = 40) of de controleconditie (n = 40).

Beschrijving
Inclusiecriteria:
- Ten minste zes maanden na bestraling en/of operatie voor rectaal adenocarcinoom stadium I-III of rectosigmoïd voor kanker met een anastomose van 15 cm of lager en voltooide behandeling
- Ten minste zes maanden na bestraling en/of chemotherapie voor anale kanker.
- Heb geen bewijs van ziekte of herhaling
- Geef voor vrouwen een gemiddelde tevredenheid of minder aan over hun algehele seksuele leven (een score van "4" of lager op de vraag "Hoe tevreden was u de afgelopen 4 weken met uw algehele seksuele leven?"
- Geef voor mannen aan, en een score van "3" of lager op de vraag "Hoe zou u uw vertrouwen beoordelen dat u een erectie kunt krijgen en behouden" voor mannen) en een score van "2" of hoger op de vraag "Hoeveel stoort dit probleem je?
- Naar het oordeel van de instemmende professionele interventionisten, in staat om goed genoeg in het Engels te communiceren om met Engels sprekende personen te werken en de studiebeoordelingen te voltooien.
- Leeftijd 21 jaar of ouder.
Uitsluitingscriteria:
- Significante cognitieve of psychiatrische stoornis die naar het oordeel van de onderzoeker voldoende is om deelname aan een psychotherapeutische interventie uit te sluiten.
- Neemt actief deel aan protocol 06-151
- Deelname aan focusgroep of kwalitatief interview.

gebruikelijke zorg (UC), de standaardzorg die patiënten krijgen

Deelnemers gerandomiseerd naar de Usual Care-arm krijgen standaardzorg.
Voor vrouwen is dit gedefinieerd als de zorg die deelnemers die niet deelnemen aan de interventie ontvangen; we zullen deze deelnemers vragen welke psychosociale zorg ze krijgen om de gebruikelijke zorg bij te houden.
Voor mannen zal "gebruikelijke zorg" bestaan uit een gestandaardiseerde verwijzing naar het mannenprogramma voor seksuele geneeskunde op MSKCC en schriftelijke informatie in de vorm van het boekje van de American Cancer Society (ACS) over seksualiteit na kanker.
Individuen die zijn gerandomiseerd naar de gebruikelijke zorg, krijgen geen aanvullende counselingsessies van de onderzoeksinterventie totdat ze klaar zijn met de vervolggesprekken en daarna wordt het hen aangeboden.
Follow-upvragenlijsten zullen alle studiedeelnemers 4 en 8 maanden na baseline beoordelen.

Kankeroverlevingsinterventie - Seksuele gezondheid (CSI-SH) plus gebruikelijke zorg (VS)
|
Het gaat om vier individuele sessies van 1 uur met drie extra telefonische booster/review-sessies tussen de vier sessies.
Sessies kunnen persoonlijk of telefonisch worden afgerond.
Hoewel alles in het werk zal worden gesteld om de boostersessie tussen de counselingsessies door te voltooien, kunnen deze worden overgeslagen als de patiënt niet beschikbaar is om de boostersessies af te ronden.
Sessie één en twee zullen gericht zijn op revalidatietechnieken en hulpmiddelen die tijdens deze eerste twee sessies met de deelnemers worden aangereikt en besproken.

It is composed of 30-items, which factor into scales that evaluate physical functioning and role functioning, as well as emotional, social, and cognitive functioning and global QOL.
Three symptom scales measure fatigue, pain, and emesis, while six single items assess financial impact, dyspnea, sleep disturbance, appetite, diarrhea, and constipation.
