Karakterisering av akut träningsrespons vid restriktiva ätstörningar

Individer med ätstörningar (ED) representerar en högprioriterad klinisk population, med bland de högsta dödlighetssiffrorna av någon psykiatrisk störning, och driven träning (DEx) är ett symtom som påvisas hos upp till 80 % av dem med ED som ökar funktionsnedsättningen och negativt påverkar behandlingsresultatet. Ätstörningar (ED) är bland de dödligaste av alla psykiatriska störningar. Emellertid är förstahandsbehandlingar för ED:er effektiva för endast ungefär 40 % av tonårspatienterna. Ett hinder för behandlingseffektivitet är ihållande driven träning (DEx; träning på ett tvångsmässigt sätt för att kontrollera vikten och/eller reglera känslor), ett kännetecknande symptom som rapporterats bland många ungdomar som diagnostiserats med restriktiva ED: er (59-73%). Bland andra negativa följdsjukdomar är DEx associerat med suicidalt beteende, sämre behandlingsresultat, ökad komorbid psykopatologi och risk för återfall. Vidare, medan flera ätrelaterade uppgifter har validerats som skiljer mellan individer med ED och friska populationer, finns det för närvarande inga välvaliderade uppgifter som direkt bedömer känslighet och känslighet för träning i ED-populationer. Det föreslagna projektet kommer att rekrytera tonåringar och unga vuxna kvinnor (16-22y) med (n = 67) och utan (n = 33) ED från två platser för att delta i en omfattande utvärdering av akut träningsrespons. Det aktuella projektet kommer 1) att karakterisera variationen i biobeteenderespons på exponering i laboratoriet för måttlig intensiv träning bland de med och utan ED; 2) skapa och förfina uppgifter som fångar engagemang av belönings- och hotrelaterade system under träning; och 3) testa hypoteser om att akut träningsrespons är mer uttalad bland individer med ED och att akut träningsrespons relaterar till ED-symptom, DEx och frilevande aktivitet. Detaljerad redogörelse för akuta träningseffekter bland individer med EDs i en kontrollerad miljö kommer att ha betydande inverkan på den empiriska studien av EDs, specifikt för att (i) förbättra bedömningen av DEx-risk och funktion (ii) belysa en testbar modell av DEx-risk och (iii) ) föreslå mål för DEx-intervention. Metoderna som validerats i detta projekt kommer att förbättra förståelsen för DEx, en behandlingsresistent ED-funktion. Study participants undergo Prescribed Exercise, Self-Paced Exercise, and Rest conditions over the course of 3 study days

ED severity, biological and affective markers of physical activity response, and both exercise-specific and general positive (reward) and negative (avoidance/escape) valence sensitivities associate with physical activity will be comprehensively examined.

During three different days, participants will:

  • A: Complete 2 different tasks, a reward task (prescribed rest condition) with self-report measures and two blood draws before and after completing the 30 minute rest condition, and a threat task (self-paced exercise condition) with self-report measures, a food challenge (to drink a milkshake) and 30 minutes of exercise.
  • B: Complete a reward task (prescribe exercise condition) where participants will earn points for exercise and have 2 blood draws before and after completing 30 minutes of exercise
  • C: Complete a threat task (self-paced rest condition) with self-report measures, a food challenge, and 30 minutes of rest.
Experimentell: Participants with Eating Disorders
Study participants undergo Prescribed Exercise, Self-Paced Exercise, and Rest conditions over the course of 3 study days
