제한적 섭식 장애의 급성 운동 반응 특성화
섭식 장애(ED)가 있는 개인은 정신 질환 중 사망률이 가장 높은 우선 순위가 높은 임상 집단을 대표하며, 추진력 있는 운동(DEx)은 장애를 증가시키고 부정적으로 작용하는 ED 환자의 최대 80%에서 입증되는 증상입니다. 치료 결과에 영향을 미칩니다.
이 연구는 발기부전이 있는 청소년 및 젊은 성인 여성의 운동에 대한 인지적, 정서적, 생물학적 반응을 특성화하고 급성 운동 반응이 자유 생활 활동 및 DEx와 연관되는지 여부를 결정하는 작업을 개발할 것입니다.
이 R21 프로젝트는 운동 주도 증상을 보다 정확하게 타겟팅하고 궁극적으로 발기부전과 관련된 임상 결과를 개선하는 치료법의 연구 및 개발을 안내하기 위한 기초 데이터를 제공할 것입니다.
연구 개요
상세 설명
섭식 장애(ED)는 모든 정신 장애 중에서 가장 치명적인 장애 중 하나입니다.
그러나 ED에 대한 1차 치료법은 청소년 환자의 약 40%에게만 효과적입니다.
치료 효능에 대한 한 가지 장벽은 제한된 운동(DEx; 체중 조절 및/또는 감정 조절을 위해 강박적인 방식으로 운동)의 지속성이며, 이는 제한적인 ED로 진단된 많은 청소년(59-73%) 사이에서 보고된 특징적인 증상입니다.
다른 부정적인 후유증 중에서 DEx는 자살 행동, 치료 결과 불량, 동반 정신 병리학 증가 및 재발 위험과 관련이 있습니다.
또한, ED가 있는 개인과 건강한 인구를 구별하는 여러 식사 관련 작업이 검증되었지만 현재 ED 인구에서 운동에 대한 민감도와 반응성을 직접 평가하는 잘 검증된 작업은 없습니다.
DEx는 ED 유지 관리에 중요한 역할을 하는 잘 이해되지 않은 증상이므로 생체 행동 기반을 조사하는 것이 매우 중요합니다.
운동의 보상 및 위협 기능에 대한 기존 연구 포인트는 ED가 있는 사람들 사이에서 DEx를 유도하고 영속시킬 수 있습니다.
ED가 있는 사람들 사이에서 운동의 보상 및 위협 기능을 모두 평가하면 이 ED 기능에 대한 기계적 이해가 향상됩니다. 궁극적으로 DEx에 대한 이해가 향상되면 치료 목표에 대한 정보를 얻을 수 있습니다.
제안된 프로젝트는 급성 운동 반응에 대한 포괄적인 평가에 참여하기 위해 두 사이트에서 응급실이 있는(n = 67) 및 없는(n = 33) 청소년 및 젊은 성인 여성(16-22세)을 모집합니다.
현재 프로젝트는 1) ED가 있거나 없는 사람들 사이에서 중간 강도 운동에 대한 실험실 노출에 대한 생체 행동 반응의 가변성을 특성화합니다. 2) 훈련 중 보상 및 위협 관련 시스템의 참여를 포착하는 작업을 생성하고 개선합니다. 3) 급성 운동 반응은 ED가 있는 개인 사이에서 더 뚜렷하고 급성 운동 반응은 ED 증상 심각도, DEx 및 자유 생활 활동과 관련이 있다는 가설을 테스트합니다.
통제된 환경에서 ED가 있는 개인의 급성 운동 효과에 대한 자세한 설명은 특히 (i) DEx 위험 및 기능 평가를 개선하고 (ii) 테스트 가능한 DEx 위험 모델을 밝히고 (iii) ED의 실증적 연구에 중요한 영향을 미칠 것입니다. ) DEx 개입 목표를 제안합니다.
이 프로젝트에서 검증된 방법은 치료 저항성 ED 기능인 DEx에 대한 이해를 향상시킬 것입니다.

- ED 참가자는 체중과 체형 문제를 특징으로 하는 DSM-5 제한 스펙트럼 섭식 장애, 즉 신경성 식욕부진(AN)에 대한 전체 기준을 충족해야 합니다. 경증, 중등도 또는 부분 완화), 비정형 AN 및 OSFED -AN 유형만 해당)
- 응급실 참가자는 의학적으로 안정되어 있어야 하며 외래 치료에 적합해야 합니다.
- 생물학적 여성, 출생 시 할당된 여성

제외 기준:
운동을 할 수 없는 질병(예: 중증 천식)이 있는 분은 금기 사항이 있는 경우 제외됩니다.

- 신체 활동과 관련된 모든 승인된 의학적 문제(심혈관, 폐 또는 대사 질환에 관한 전화 선별 질문에서 조사됨).
