背景:
川崎综合症是一种急性发热性疾病,主要发生在以前健康的幼儿中。 它的病因不明,1967 年在日本首次被描述。 这种疾病的急性死亡率约为 3%。 美国疾病控制中心将川崎综合症定义为持续五天或更长时间且无法找到任何解释的发烧。 患者还必须至少有以下四种症状: 双侧结膜感染;嘴唇、咽部感染或裂开或“草莓舌”;手掌或脚底红斑,或手或脚水肿,或全身或甲周脱屑;皮疹;和颈部淋巴结肿大。
冠状动脉瘤发生在 15-20% 的患病儿童中。 过去,没有任何治疗被证明可以有效预防这种并发症。 日本的研究人员开始使用 IVGG 来减少动脉瘤的形成。 显示该疗法有用性的初步结果导致在日本进行了一项多中心试验,其中为同时接受阿司匹林治疗的儿童给予 400 mg/kg/天的 IVGG,持续 5 天。 日本试验的结果显示,在疾病发作后 29 天内,42% 接受阿司匹林治疗的儿童发生冠状动脉扩张,15% 接受 IVGG 和阿司匹林治疗的儿童发生冠状动脉扩张。
设计叙述:
第一阶段是随机的、非盲的并且按年龄、性别和中心分层。 受试者被随机分配至患病第 14 天接受 80 至 120 mg/kg/天的阿司匹林,随后减至 3 至 5 mg/kg/天的单次每日剂量或减至 400 mg/kg/天的静脉注射丙种球蛋白连续四天加阿司匹林如上所述。 主要终点是超声心动图显示的动脉瘤形成。 随访时间为 7 周。
II 期试验于 1986 年 5 月开始招募 549 名患者,并于 1989 年 11 月结束招募。 276 名儿童被随机分配到连续四天接受 400 mg/kg 静脉内丙种球蛋白。 273 人在 10 小时内接受了 2 g/kg 体重的单次输注。 两个治疗组在第 14 天之前每天接受 100 mg/kg 的阿司匹林,然后每天接受 3 至 5 mg/kg。 主要结果变量是在两周和七周的随访检查中是否存在明显的冠状动脉异常。 在两周的访问中获得了 523 名儿童的超声心动图,在七周的访问中获得了 520 名儿童的超声心动图。
