尽管据报道 CPVA 的成功率更高(高达 90%),但之前报道的大多数 CPVA 程序包括左心房线性消融 (LALA)、额外的消融损伤或将二尖瓣连接到后肺静脉或沿着二尖瓣的线左心房的屋顶。 因此,CPVA 的成功可能部分归因于大量心房组织的消融 (6-7)。 然而,额外的消融 (LALA) 可能导致潜在的促心律失常作用,从而导致非典型左心房扑动的发展 (8)。
在 Arentz 等人 (9) 的一项研究中,与 SPVI 相比,CPVA 具有更高的成功率和更短的透视时间。 然而,该研究包括同时患有 PAF 和持续性 AF 的受试者,而那些患有持续性 AF 的受试者可能具有不同的潜在机制。 此外,另外两项比较 PVI 和 CPVA 疗效的随机研究显示了相互矛盾的结果 (10-11)。
因此,我们将启动这项随机对照研究,以评估 CPVA 与 SPVI 在仅接受阵发性房颤消融的受试者中的疗效。

描述
纳入标准:
- 阵发性房颤患者,其发作时间从未超过 48 小时
- 具有消融临床指征的患者
- 18岁以上的患者。
- 患者愿意并能够给予知情同意。
- 允许并发心房颤动和心房扑动的患者。
排除标准:
- 患有会限制整个研究期间参与的医疗条件的患者。
- 怀孕的患者(所有有生育能力的女性必须在入组后 1 周内进行阴性妊娠试验)。
- 既往接受过 AF 消融的患者

有源比较器:环肺静脉消融术
