该试验是一项开放标签、随机、对照试验,与标准抗生素疗法相比,评估粪便微生物群移植 (FMT) 治疗复发性艰难梭菌感染 (CDI) 的安全性和有效性。 在过去十年中,艰难梭菌感染 (CDI) 的发病率和严重程度有所增加,并且与不良后果相关,包括发病率、死亡率和医疗保健费用的增加 (1-8)。 第一次 CDI 发作后,15-35% 的患者会复发,而有一次复发的患者中有 45-65% 会再次复发 (9, 10)。 生态失调 - 粪便微生物组多样性降低 - 被认为是导致高复发率的原因 (11)。 FMT 通过输注来自健康供体的液体粪便制剂,快速成功地恢复了患病患者的正常肠道微生物。 在系统评价和荟萃分析中报告的约 90% 的病例中,FMT 导致疾病消退,没有发现任何显着的不良事件 (12, 13)。
NorthShore 系统中年龄 > 18 岁且被诊断患有活动性 CDI 的所有住院患者(定义为每天 > 3 次腹泻粪便和艰难梭菌聚合酶链反应 (PCR) 检测呈阳性)将接受评估以纳入研究。 在 CDI 指数发作后 15 至 90 天内出现首次或多次 CDI 复发的住院患者将有资格入组。 排除标准将包括怀孕、中性粒细胞减少症(绝对中性粒细胞计数 <1000/μl)、保留灌肠禁忌症或供体饮食中未控制的食物过敏。 符合条件的患者将接受书面知情同意,然后随机分配到干预组和对照组。

两位此前曾为 FMT 捐献过粪便的健康“万能”捐献者表示愿意参与该研究。 捐助者将完成美国血库协会捐助者问卷调查,了解接触传染源的情况,并接受血清学和粪便检测以检测传染病或致病菌/病毒,如前所述 (17)。

- 活动性艰难梭菌感染的诊断,定义为每天 > 3 次腹泻粪便和艰难梭菌 PCR 检测呈阳性
