肾损伤是骨髓瘤的一种危及生命的病症。 20-25% 的患者在诊断时会出现肾功能障碍。 由于早期死亡率高,结果不佳,在伴有肾功能衰竭的骨髓瘤试验中,28% 的新诊断骨髓瘤患者无法存活超过 100 天,而总体而言这一比例为 10%。
本研究旨在建立:
- 无论是基于蛋白体抑制(硼替佐米)还是基于免疫调节(沙利度胺)的疗法,都能在绝大多数患者中实现血清游离轻链 (sFLC) 的阈值降低。
- sFLC 对前 2 个周期的反应(早期反应者)是否可以预测对接下来的 2 个治疗周期的血液学和肾脏反应。

描述
纳入标准:
- 参与者愿意并且能够对参与试验给予知情同意。
- 在 NHS(国民健康服务)血液肿瘤中心就诊的患者。
- 新诊断的症状性骨髓瘤患者。
- 肾小球滤过率 (GFR) <30 毫升/分钟。
- 慢性肾脏病 (CKD) 分期基于估计或测量的 GFR。 CKD 4 期(15-29 ml/min)和 CKD 5 期(<15 ml/min)有资格进入研究。 预计中心将考虑在这组患者中使用液体复苏和脉冲剂量的类固醇治疗,以在试验筛选之前挽救肾功能。
- 许多新诊断出骨髓瘤和肾功能衰竭的患者将有导致肾损伤的既往疾病(高血压、糖尿病等)。 如果有医疗状况(例如 高血压、糖尿病)可能导致肾损伤,在之前的稳态和研究筛选之间必须有进一步的下降(≥15 mls/min GFR)。

排除标准:

- 研究者认为可能使参与者因参与试验而面临风险,或可能影响试验结果或参与者参与试验的能力的任何其他重大疾病或病症。
- 任何以下实验室异常:
- 中性粒细胞绝对计数 (ANC) < 1.0 x10^9/L
- 血小板计数 <75 x 10^9/L
- 血清 SGOT/AST 或 SGPT/ALT(血清谷氨酸草酰乙酸转氨酶/天冬氨酸转氨酶或血清谷氨酸丙酮酸转氨酶/丙氨酸转氨酶)>3 x 正常值上限。
- 在进入试验前 14 天使用任何标准/实验性抗骨髓瘤药物治疗,不包括地塞米松。
- CKD 阶段 < 4。
- 打算使用物理方法去除血清游离轻链,例如血浆置换或高截留透析。
- 2 级或以上的神经病变(美国国家癌症研究所不良事件通用术语标准 (NCI CTCAE) v 4.0)将排除沙利度胺和硼替佐米的使用。
