患者将通过初级保健和二级保健信托基金招募。 在初步筛选访问以确定受试者的资格后,他们将被随机分配到 EndoBarrier 或控制组(常规药物治疗,包括饮食咨询),并被邀请在 24 个月内进行 15 次研究访问。 标准药物治疗药物滴定将根据美国糖尿病协会的指南进行。 之所以选择这些指南,是因为它们适用于国际受众,因此将遵守当前的全球最佳实践,在研究完成后公布该试验的结果时,这些实践仍可能具有相关性。

本研究的主要结果是比较 EndoBarrier 装置与传统药物疗法、饮食和运动对肥胖相关 2 型糖尿病的治疗效果,以及它们在以下方面的有效性:

将在访问 1(筛选访问)时执行以下评估:
知情同意、纳入和排除标准、人口统计学、病史、体格检查、身体测量包括身高、体重和腰围、生命体征、心电图、尿白蛋白肌酐比、常规血液学和生化、C13尿素呼气试验和尿妊娠试验,如果适用。 如果受试者同意参加亚组机理研究,将完成额外的检查。

两组都将进行以下测量:

EndoBarrier 组:

手术后(最多 12 小时)受试者可能需要在医院过夜,以便医生监测他们的状况,但这通常不是必需的。
在插入期间,荧光透视将用于确定装置的位置。 记录透视图像的视频和照片以帮助研究人员做出治疗决定,并可能传递给研究赞助商。 这些图像将不包含任何可识别患者身份的数据,但将仅标有姓名首字母和研究参与编号。
在访问 11(12 个月)时,EndoBarrier 装置将被移除。 移除程序与放置程序相当,但通常需要更少的时间。 在设备移除程序之前,受试者将接受全身麻醉。 在移除设备期间,将内窥镜插入口腔并引导至小肠,以便医生对该区域进行目视检查。 将导丝插入内窥镜。 内窥镜的末端有一个小钩子,医生会用它把设备拉回内窥镜。 然后通过口腔取出包含该装置的内窥镜。 然后医生将再次插入内窥镜以检查放置设备的区域。 手术后,受试者可以回家。
EndoBarrier 组中的所有受试者将在第 2、6、7、9、11、12、13 和 15 次研究访视时与胃肠病学家或同等专家会面,讨论他们的健康状况以及有关 EndoBarrier 植入物的任何问题。 EndoBarrier 患者还将接受糖尿病专家的频繁审查,以审查他们的药物治疗方案(访问 2、6、7、9、12、13 和 15)。

- 年龄18-65岁(男或女)
- T2DM 至少 1 年(HbA1c 7.5-10.0% = 58-86 毫摩尔/摩尔)
