纳入标准:
- 年龄≥18岁
- 已根据主治医师的指示开具预防性亭扎肝素(3500 IU 或 4500 IU)处方
- 患者因医疗原因被以下病房之一(但不限于)收治:肾脏科、内科、心脏病科或呼吸科
- 慢性严重肾功能损害定义为 eGFR ≤ 30 mL/min/1.73 m2 在处方时和可用时,基线时的 eGFR,即 ≤ 30 ml/min/1.73 过去 3 个月的 m2

排除标准:
