신장애 환자에게 예방적 투여량을 투여했을 때 틴자파린의 생체 축적에 관한 연구 (STRIP)
이 연구의 목적은 비수술적 조건에 대한 혈전 예방이 필요한 중증 만성 신장 질환(CKD) 환자에서 예방적 용량의 tinzaparin을 매일 반복 투여한 후 항-Xa 활성의 축적이 발생하는지 평가하는 것입니다.
정맥 혈전색전증(VTE)의 위험이 있는 중증 CKD 환자(eGFR ≤ 30 ml/min /1.73 m2)에서 혈전 예방을 위해 고정 용량으로 사용되는 틴자파린은 상당한 수준으로 생체 축적되지 않을 것으로 예상되며, 이는 20% 이상의 증가를 의미합니다. 항 Xa 평균 수준의 2일 또는 3일과 5일 사이.

만성 신장 질환(기준 eGFR ≤30 ml/min/1.73)으로 진료과(예: 신장, 호흡기, 순환기내과 및 내과)에서 비수술적 이유로 입원한 피험자 m2) 1일 1회 3500 IU(또는 BMI > 30kg/m2인 경우 4500 IU) 용량으로 틴자파린 예방요법을 받음.
약동학 매개변수: 치료 2일(또는 3일), 5일 및 8일 후에 수행된 최고 항-Xa 분석 및 5일째에 한 번의 최저 항-Xa 분석. 틴자파린의 생체 축적은 이 투여 요법이 치료 과정에 걸쳐 항-Xa 수준의 과도한 증가와 관련되는지 여부를 결정함으로써 평가될 것입니다(통계 분석에 대해서는 아래 참조).
채용 과정:
잠재적인 연구 주제에 대한 체계적인 선별은 4개의 서로 다른 소프트웨어를 통해 주요 컴퓨터 검색 필드를 기반으로 연구 코디네이터에 의해 이루어집니다. 이러한 데이터베이스를 통해 연구원은 궁극적으로 치료를 받을 입원 환자를 식별하여 적격한 피험자의 예비 선택을 수행할 수 있습니다. 약국 부서에서 틴자파린 처방의 검증을 담당하는 약사는 적격 후보를 식별하여 초기 심사에서 연구팀과 협력할 것입니다. 처방된 약제의 선택은 처방자의 재량에 따르므로 모집 절차는 주로 혈전 예방 용량의 틴자파린 처방이 작성되는 즉시 시작됩니다. 포함 기준을 충족하고 제외 기준을 충족하지 않는 피험자는 적격 후보로 간주됩니다. 인종 및 성별과 같은 추가 기본 정보가 해당 시점에 수집됩니다. 적격 참가자는 연구팀의 구성원이 직접 해결합니다. 사전 동의는 병원 윤리 위원회에서 이미 승인한 정보 양식 및 사전 동의(FIC)를 통해 구하고 문서화됩니다.
데이터 수집:
4개의 혈액 샘플 중 3개는 틴자파린 주사 후 4±1시간 후에 2일(또는 3일), 5일 및 8일에 수집되고 1개의 최저점은 5일에 수집됩니다.
미리 식별된 튜브(인코딩됨)는 혈액 샘플 조작 및 실험실로의 배송에 대한 구체적인 지침과 함께 간호사 및 샘플 배송 직원에게 제공됩니다.
연구 샘플:
본 연구에서는 anti-Xa의 Cmax가 20% 이상 증가하면 생체축적이 통계적으로 유의하다고 판단된다. 인구 규모의 추정은 Mahé et al.에서 얻은 정보를 사용하여 수행되었습니다. 연구 대상 인구의 특성이 비슷하기 때문입니다. 평균 혈장 항-Xa 피크 수준에 관해서는 Bara L et al. 혈전 예방을 위해 tinzaparin 3,500 IU를 투여받은 431명의 환자는 평균 0.15 IU/ml를 나타냈으며, 이는 본 연구에서 대부분의 피험자에게 투여될 용량입니다. - 연령 ≥ 18세
- 치료 의사의 지시에 따라 예방적 틴자파린(3500 IU 또는 4500 IU) 처방이 시작되었습니다.
- 신장내과, 내과, 심장내과, 호흡기내과 병동 중 한 병동(이에 국한되지 않음)에 의학적 이유로 입원한 환자
- eGFR ≤ 30 mL/min/1.73으로 정의되는 만성 중증 신장애 처방 당시의 m2 및 사용 가능한 경우 기준선의 eGFR, 즉 ≤ 30 ml/min/1.73 지난 3개월 동안 m2
- 예상 체류 기간 ≥ 5일
- tinzaparin의 두 번째 또는 세 번째 투여 후 최대 3 ± 1시간 이내에 얻은 서면 동의서.
제외 기준:
- 초비만(체질량지수(BMI) > 50kg/m2)
- UFH, LMWH 또는 경구 인자 Xa 억제제로 틴자파린의 첫 번째 용량을 시작하기 48시간 미만 전에 치료
- tinzaparin의 첫 번째 용량을 시작하기 48시간 전에 tinzaparin 이외의 LMWH로 예방
- tinzaparin의 첫 번째 용량을 시작하기 12시간 전에 헤파린으로 예방
- 아르가트로반, 비발리루딘 치료 < 틴자파린 첫 투여 시작 24시간 전
- 틴자파린의 첫 번째 용량을 시작하기 전 7일 미만 동안 경구용 직접 트롬빈 억제제, 다나파로이드, 폰다파리눅스 또는 항비타민 K 제제로 치료
- 베이스라인 eGFR > 30 ml/min/1.73인 개인의 급성 신부전 m2
- 포함 전 72시간 이상 사용 중인 예방적 틴자파린
- 중증 간부전(Child-Pugh C)
