插管和机械通气是治疗创伤性脑损伤 (TBI) 患者的常见治疗方法。 插管可以控制气道并有助于清除呼吸道分泌物。 机械通气允许控制动脉二氧化碳以帮助控制颅内压。 最近的证据表明,肺保护性通气(潮气量为 6 ml/kg 预测体重和适度的呼气末正压)可改善脑损伤后的结果并减少脑肺串扰。
TBI 呼吸衰竭的治疗必须平衡改善肺功能的需要与胸腔内压升高对平均动脉压、颅内压和静脉回流的负面影响。 增加呼气末正压 (PEEP) 和平均气道压力的传统治疗代表了相互竞争的利益。 需要改善动脉氧合作用同时避免负面血液动力学影响的方法。
仅在少数临床研究中研究了头部受伤对呼吸力学的影响。 (1-3) 值得注意的是,其中最早指出 TBI 后的通气灌注 (V/Q) 匹配不是肺萎陷或肺实质疾病的结果,而是继发于灌注改变。 这一发现存在三种可能性:
- 局部灌注的重新分布,部分由下丘脑介导

- 血流动力学不稳定(收缩压 < 100 mm Hg,心律失常)
- 不受控制的颅内压 (> 20 mm Hg)
- 脊髓损伤伴低血压
- 严重急性呼吸窘迫综合征 (ARDS) (PaO2/FiO2 < 100)
- 胸部简略损伤评分 (AIS) > 3
