卡维地洛联合小檗碱对肝硬化 GOV 影响的内镜和微生物学评估:一项前瞻性队列研究
卡维地洛已被证明在降低门脉高压方面比普萘洛尔更有效。
大量研究表明,菌群失调与门静脉高压症的进展互为因果。
小檗碱具有调节肠道菌群的作用。本研究通过观察胃镜、超声内镜和肠道菌群的变化,评价卡维地洛和小檗碱降低门静脉压力的作用。

详细说明
伴有中、重度食管胃底静脉曲张的肝硬化患者发生食管胃底静脉曲张出血的风险较高。
一旦发生食管胃底静脉曲张出血死亡率很高,会给家庭和社会造成很大的损失。
因此,通过经济有效的方法预防食管胃底静脉曲张出血具有重要的社会和经济意义。
作为第三代NSBB,卡维地洛在降低HVPG方面比普萘洛尔更有效,被Baveno VI推荐为EVB一级预防的一线药物。
大量研究表明,菌群失调与门静脉高压症的进展互为因果。
应用卡维地洛可降低门静脉压力,门静脉高压症改善后,肠粘膜屏障和菌群失调也会发生改变。
小檗碱能调节肠道菌群,临床应用安全有效。
胃镜检查仍是筛查静脉曲张的主要方法。
我们可以确定患者是否存在 GOV,并评估静脉曲张出血的风险。
但胃镜只能观察消化道管腔内的情况,超声内镜可以扫描管壁外部,从而更全面地评估门静脉高压症的变化。
本研究旨在将超声内镜应用于研究的全过程,作为更灵敏的观察门脉高压变化的手段,系统评价门脉高压的持续变化。
为更全面、科学地评价门静脉高压症提供理论依据和测量手段。本研究将需要一级预防的GOV患者随机分为两组。
分别给予卡维地洛或卡维地洛联合黄连素12个月。
通过ES和EUS判断静脉曲张缓解程度,同时检测口腔和肠道菌群的变化,以了解卡维地洛和小檗碱降低门静脉高压症的可能机制。

描述
纳入标准:
基于病理或临床诊断的HBV相关或/和HCV相关肝硬化患者;
- 抗病毒治疗;
- 男女不限;
- ES显示存在食管和胃底静脉曲张和/或红色体征;
- Child-Pugh < 10,并且 meld < 29;
- 签署知情同意书。
排除标准:
• 2 周内使用过抗生素、益生元、益生菌和质子泵抑制剂;
- β受体阻滞剂的任何禁忌症包括哮喘、慢性阻塞性肺病、过敏性鼻炎、NYHA(纽约心脏协会)IV级心力衰竭、房室传导阻滞、窦性心动过缓(HR < 50/分钟)、心源性休克、低血压(SBP < 85mmHg)、病态窦房结综合征、胰岛素依赖型糖尿病、外周血管疾病;
- 不稳定的高血压和长期从事驾驶;
- 任何影响生存的恶性肿瘤,不包括已治愈的;
- 门静脉血栓患者;
- PT延长大于4秒,PLT<30×10^9/L;
- 孕妇和哺乳期患者;
- 门静脉高压症手术史;既往EVL(内镜下静脉曲张结扎术)或硬化剂治疗史,门静脉高压症手术史,包括门体分流术、脾切除术和经颈静脉肝内门体分流术;
- 心、肺、肾、脑、血液、神经系统等重要脏器严重疾病患者;
- 对卡维地洛和小檗碱过敏;
- 严重的全身性疾病;
- 溶血性贫血和缺乏葡萄糖 - 6-磷酸脱氢酶患者
- 拒绝参与研究。
