重症监护病房幸存者坐姿干预的效果:一项随机对照试验
“从坐到站”是独立生活的关键。 对于重症监护病房 (ICU) 的幸存者,未能执行从坐到站的动作会导致卧床不起,无法参与重要的日常生活活动 (ADL) 或工具性 ADL。 最近的研究表明,31% 的 ICU 幸存者在回家后仍然卧床不起,无法“坐立”。 我们的初步研究结果进一步表明,70% 患有 ICU 获得性无力 (ICU-AW) 的 ICU 幸存者在 ICU 出院后一个月无法“坐起”。
这个为期 3 年的研究项目的目的是开发一种可行且有效的“坐到站”护理(STS 护理)。 提出了一项随机对照试验 (RCT) 设计,以检验 STS 护理在改善 ICU 患者的“从坐到站”能力、身体机能以及 ICU 出院后一年的卧床率和死亡率方面的效果。
该试验在入组前由研究地点的人类研究伦理委员会批准。 连续入住大学附属医疗中心的六个内科 ICU 的成年 ICU 患者(≥ 20 岁)如果在 ICU 出院时无法独立“坐起”,则符合本研究的条件。 在签署知情同意书后,参与者将首先根据“能够保持直立而不跌倒”或“无法保持直立”进行分层,然后根据计算机生成的随机化表格随机分为干预组或常规护理组。 干预组的参与者接受常规护理和 STS 护理。

对于这项分层随机和单盲研究,从台大医院药物重症监护病房出院 48 小时后被筛选为坐立能力失败的参与者(≧20 岁)将被纳入研究。 在分配过程中,参与者将按稳定的坐姿和无法稳定的坐姿进行分层,然后将参与者随机分配到实验组和对照组。 实验组将接受常规护理和额外的 STS 床边护理方案,每天一次,持续两周或在两周内出院。 相反,对照组会接受常规护理。
所有参与者在 ICU 出院后将在 5 个时间点进行为期一年的随访:48 小时、14 天、1、3、12 个月。 综合功能评估(坐立成功率、FSS-ICU评分、MRC肌肉量表评分、膝伸肌肌力、巴特尔日常生活活动指数(ADL)评分、日常器械活动评分将执行生活量表 (IADL)、6 分钟步行测试的步行距离、30 秒内坐到站的重复次数、卧床率、死亡率)。 估计有 206 名参与者将在 ICU 出院一年后被招募和随访。
