「座ったり立ったり」は自立生活の鍵です。 集中治療室 (ICU) の生存者は、座位から立位を実行できないと寝たきり状態になり、重要な日常生活活動 (ADL) や手段的 ADL に参加できなくなります。 最近の研究によると、ICU生存者の31%は帰宅後も寝たきりで「座ったり立ったり」することができなかった。 我々の予備調査結果はさらに、ICU後天性衰弱(ICU-AW)を患ったICU生存者の70%が、ICU退室後1か月で「座ったり立ったり」することができなくなったことを示した。
この 3 年間の研究プロジェクトの目的は、実現可能で効果的な「座位から立位」ケア (STS ケア) を開発することでした。 ICU 患者の「座って立つ」能力、身体機能、ICU 退院後 1 年後の寝たきり率と死亡率の改善における STS ケアの効果を調べるために、ランダム化比較試験 (RCT) デザインが提案されています。
この試験は、登録前に研究施設のヒト研究倫理委員会によって承認されました。 大学関連医療センターの6つの医療ICUに連続して入院した成人ICU患者(20歳以上)は、ICU退院時に自立して「座ったり立ったり」できない場合に研究の対象となった。 インフォームドコンセントに署名すると、参加者はまず「転倒せずに直立姿勢を維持できる」か「直立姿勢を維持できない」かで層別化され、次にコンピューターで生成されたランダム化表に従って、介入グループと通常のケアグループに別々にランダム化されます。 介入グループの参加者は通常のケアと STS ケアの両方を受けました。

この層化無作為化および単一盲検試験では、国立台湾大学病院の投薬ICUから48時間退室後、座位から立位に失敗したとスクリーニングされた参加者(≧20歳)が研究に登録されます。 割り当て手順では、参加者を座位姿勢が安定している場合と座位姿勢が安定していない場合で層別化し、参加者を実験群と対照群にランダムに割り当てます。 実験グループは、定期的なケアと追加の STS ベッドサイドケアプロトコルを 1 日 1 回 2 週間受け入れるか、2 週間以内に退院することになります。 対照的に、対照群は定期的なケアを受け入れます。

- ICU退院までに座ったり立ったりすることができなくなります。
除外基準:
- コマンドに従えません。
- 入院前は寝たきり。
- ICU退室後は人工呼吸器に依存。
- 緩和ケアを受けられました。
- 神経筋または筋骨格の病因により歩行できない体幹または下肢の併存疾患(例: CVA、脊髄損傷、下肢の切断または骨折)。
- 飛沫や接触に注意して置かれる(例: 結核、SARS、新型コロナウイルス感染症(COVID-19)を引き起こす)
