纳入标准:
- 根据 2016 年 WHO 分类,经组织学或病理学确诊的 AML。 患有复杂核型 AML (CK-AML) 和 TP53 突变 AML 的患者有资格参加本研究。
- AML 在一线治疗后复发或难治,最多三线治疗。 具有 FLT3 或 IDH1/IDH2 突变的 AML 患者应接受至少一种可用的 FLT3 或 IDH1/IDH2 抑制剂
- 年满 18 岁
- ECOG 体能状态 ≤ 2
- 如果干细胞输注后 ≥ 30 天,没有移植物抗宿主病 (GVHD) > 1 级的证据,并且停止所有免疫抑制治疗 ≥ 10 天,则接受同种异体 HSCT 的患者符合条件
- 先前的细胞毒性化疗必须在研究治疗的第 1 天前至少 10 天完成,并且由于较早给予的药物引起的所有 AE(不包括脱发、痤疮、皮疹)应该已经恢复到 < 1 级。预计患有血液系统恶性肿瘤的患者在进入研究时有血液学异常。 这些异常被认为主要与潜在的白血病有关,不被视为毒性 (AE),不需要解决到 < 1 级

患者必须具有以下定义的足够器官功能:

- AST(SGOT)/ALT(SGPT) ≤ 3X ULN(除非归因于肝脏的白血病浸润)
- 碱性磷酸酶≤5X ULN

- 有中枢神经系统白血病病史的患者在第 1 天治疗前必须稳定且 CSF 清晰 > 2 个月(患者可以接受鞘内维持化疗)
- 有生育能力的女性患者必须在入组前 1 周内进行阴性妊娠试验。 性活跃的育龄女性患者和性活跃且有育龄女性伴侣的男性患者必须同意在接触研究药物期间和最后一次研究给药后 30 天内与其伴侣一起使用高效避孕方法药物。
- 能够理解并愿意签署书面知情同意书。
排除标准:
- 接受任何其他研究药物或同步化疗或免疫治疗的患者
- 患有急性早幼粒细胞白血病 (APL) 的患者经 t(15;17) 确认(即 FAB 亚型 M3 和 M3 变体)
- 预先使用任何天冬酰胺酶产品进行治疗。 接受 ≤12 周 BCL-2 抑制剂(包括维奈托克)治疗的患者符合条件。
- 绝对外周爆炸 > 100,000/μL。 在开始治疗前和研究开始治疗后最多 10 天,允许使用羟基脲控制母细胞计数。 在此期间开始使用羟基脲的决定由主治医师决定。
排除具有以下临床病史的患者:
- 与胆石症无关的严重胰腺炎。 严重急性胰腺炎的定义是脂肪酶升高 > 5X ULN 并且有体征或症状
- 无端深静脉血栓形成(DVT)
- 肺栓塞
- 出血性或血栓栓塞性中风
- 最近三个月内需要全身化疗、免疫治疗或靶向治疗的其他恶性肿瘤
- 活动性、不受控制的感染;接受积极治疗并用抗生素控制感染的患者符合条件
- 不受控制的并发疾病,包括但不限于症状性充血性心力衰竭、不稳定型心绞痛、心律失常或精神疾病/社会状况,根据研究中心首席研究员的判断,这些疾病会限制对研究要求的遵守
- 孕妇及哺乳期女性患者不同意停止哺乳。
- 不受控制的主动发作
