理由 癌症患者的营养评估通常使用传统的人体测量技术进行,该技术对短期变化不敏感,并且不能反映 FFM、FM 和水合状态的变化。 使用氧化氘稀释 (DOD) 等参考技术以及临床实践中更容易使用的现场方法(BIA 和人体测量学)评估身体成分,可以检测 HM 儿童和青少年的短期变化。

一般目标 评估接受积极治疗的 HM 儿科患者的身体成分和营养状况的变化。
具体的目标
- 在干预开始时和干预后 6 个月,使用人体测量和饮食指标评估营养状况。
- 使用参考和现场技术在基线和干预后 6 个月评估脂肪量和去脂量。
- 建立可在临床实践中常规应用的监测营养状况和身体成分的方案。
方法学 研究设计 这是一个预测试/后测试设计。 墨西哥两个地区(埃莫西约、索诺拉/瓜达拉哈拉、哈利斯科州)诊断为 HM 的 2 至 14 岁儿童将参加。
样本量:为了确定样本量,我们将假设去脂体重 (FFM) 的标准差 (SD) 为 2.19 Kg,方差为 4.8 Kg,Zα 为 1.96(p = 0.05),平均差 (°) 为 1 公斤。 样本量的计算考虑了 20% 的 Beta 误差 (B = 0.84),具体取决于同意参与的患者数量以及按照既定标准排除的患者数量。 根据上述数据,估计样本量计算为 38 名参与者,详情如下式所示。

膳食评估: ENSANUT 频率调查问卷将适用于各个年龄组:学龄前儿童、学童和青少年。
人体测量 体重和身高:测量体重(电子秤0.150+0.05公斤秤)或儿童秤(0-20公斤)。 身高将使用 SECA 身高计或婴儿计进行测量,按要求精确到 0.1 厘米。
中上臂围 (MUAC):该测量方法通过测量中点臂围来识别 5 岁以下儿童的营养不良,无需考虑体重、身高和年龄。
三头肌皮褶(TSF):参与者站立,双脚并拢,肩膀放松,手臂放松地垂在两侧。 检查者将站在患者的右侧,并将手臂背面的最高点放置在与臂围标记的中点相同的区域。 用拇指和食指轻轻抓住皮肤和皮下脂肪组织的皮肤皱襞,距离皮肤刻划点约1.0cm,皮肤皱襞平行于上臂的纵轴。 卡尺的钳口将放置在皮肤标记的水平处,这些钳口应垂直于折叠的长度放置。 轻轻握住皮褶,测量皮褶厚度,精确至 1 毫米。
腰围(WC):在站立位置和呼气后,用可伸缩卷尺在脐疤痕处测量腰围。
所有测量结果将由世界卫生组织的Anthro软件处理,用于计算体重/年龄、身高/年龄的Z分数、<60个月儿童的体重/身高、BMI-年龄-性别、儿童身高/年龄 > 60 个月。 将进行体检以观察营养不良的迹象(皮下脂肪减少、肌肉萎缩、水肿等)。
身体成分 氧化氘稀释:通过使用傅里叶变换红外光谱 (FTIR) 和唾液样本稀释氧化氘 (DOD) 来测量体内水分总量。 氘的剂量将是国际原子能机构建议用于现场研究的剂量(该剂量不会对未成年人构成风险)。
测量将在患者去洗手间后进行,将使用无菌材料进行测量,以保证测量人员的清洁和安全。 将采集给药前唾液样本,一旦根据患者体重计算出剂量,则将进行 DOD,应使用注射器采集以避免剂量溢出。 3小时后,将采集服药后唾液样本,在计算时,将考虑患者是否摄入了额外的液体。
电生物阻抗 (BIA):BIA 被认为是一种简单、快速且非侵入性的技术,可以估计体内水分总量 (TBW)、去脂体重 (FFM) 以及脂肪量 (FM)。 测量时患者必须处于仰卧位至少 5 分钟,然后再开始。 将4个电极放置在其左端,两个在手上,两个在脚上,彼此相距5cm,施加50mHz的电流,然后读取电阻和电抗读数以获得测试结果。 阻抗,遵循标准化方法。
