纳入标准:
- 年龄≥18岁,性别不限;
- 从发病到治疗的时间<4.5h;症状开始的时间定义为“它们最终表现正常的时间”;
- 临床诊断为缺血性脑卒中(诊断依据中国急性缺血性脑卒中诊治指南2018);
- 发病前MRS为0-1分
- 基线 NIHSS 5-25(均包括在内);
- 患者或代理人的知情同意。
排除标准:
- 打算进行血管内治疗;
- NIHSS意识评分>2;
- 对替奈普酶或阿替普酶过敏;
- 既往颅内出血病史;
- 3个月内有严重头部外伤或中风病史;
- 3个月内有颅内或脊柱手术史;
- 3周内有消化道或泌尿道出血史;
- 2周的大手术;
- 1周内在不易受压的止血部位进行动脉穿刺;
- 颅内肿瘤(神经外胚层肿瘤除外,如脑膜瘤)、颅内大动脉瘤;
- 颅内出血(包括脑实质出血、脑室内出血、蛛网膜下腔出血、硬膜下/硬膜外血肿等);
- 活动性内脏出血;
- 发现主动脉弓夹层;
- 积极降压治疗后,高血压仍未得到控制:收缩压≥180 mm Hg,或舒张压≥100 mm Hg;
- 有急性出血倾向,包括血小板计数低于 100×109/L 或其他;
- 血糖<2.8 mmol/L或>22.22 mmol/L;
- INR>1.7 或 PT>15 s 口服华法林抗凝剂;
- 24小时内接受肝素治疗;
- 48小时内使用凝血酶抑制剂或Xa因子抑制剂;
- 头颅CT或MRI显示大面积梗死(梗死面积>大脑中动脉的1/3);
- 癫痫发作或其他神经/精神疾病后因偏瘫(托德麻痹)而无法或不愿合作的受试者;
- 孕妇、哺乳期妇女或试验期间不同意使用有效避孕措施的受试者;
- 筛选前3个月内参加过其他临床试验;
- 研究者判断不适合或参与本研究可能导致受试者面临更大的风险。
