食管癌缺氧成像指导个体化放射治疗 (PIONEER)

描述
纳入标准:
- 年龄≥ 18 岁的男性或女性受试者。
- 有资格接受新辅助或根治性 CRT 的食管癌患者。
注意:如果多学科团队推荐 CRT,则转移性疾病负担有限的 IV 期患者可能符合条件。
- ECOG 体能状态 ≤ 2。
对于女性受试者:妊娠试验阴性或绝经后状态的证据。 绝经后状态将被定义为在没有其他医疗原因的情况下闭经 12 个月。 以下年龄特定要求适用:
50岁以下的女性:
- 停止外源性激素治疗后闭经≥12个月;和
- 该机构绝经后范围内的促黄体激素和促卵泡激素水平;或者
- 接受过绝育手术(双侧卵巢切除术或子宫切除术)。
≥ 50 岁的女性:
- 停止所有外源性激素治疗后闭经 12 个月或更长时间;或者
- 末次月经 >1 年前有过辐射诱发的更年期;或者
- 化疗引起的绝经,最后一次月经 > 1 年前;或者
- 接受过绝育手术(双侧卵巢切除术、双侧输卵管切除术或子宫切除术)。
- 从与任何先前治疗相关的毒性恢复到基线或≤ 2 级 CTCAE v5,除非 AE 在临床上不显着和/或在支持疗法中稳定。

排除标准:
因任何原因无法接受核磁共振检查,包括:
- 严重的幽闭恐惧症不适合预先用药
- 体内存在不兼容 MRI 的金属物体或植入式医疗设备(例如,不兼容 MRI 的心脏起搏器、深部脑刺激器、神经刺激器、动脉瘤夹、手术夹、假体、人造心脏、带钢部件的阀门、金属碎片、弹片、眼睛附近的纹身或钢制植入物)
- 既往或并发恶性肿瘤的患者,其自然病史或治疗不会干扰放射治疗的安全性或疗效评估,符合本试验的条件。
受试者患有严重的、不受控制的、显着的并发疾病或近期疾病,这将使他们无法成为化放疗的候选者。
- 任何其他根据研究者的判断,由于安全问题或遵守临床研究程序而禁止受试者参与临床研究的情况。
- 在筛查时已知的 HIV 感染具有可检测的病毒载量。 注意:接受有效抗逆转录病毒治疗或计划在筛选时进行抗逆转录病毒治疗的患者有资格参加本试验。 资格不需要 HIV 检测。
- 已知先前对钆、FMISO 或其配方中的任何成分严重超敏反应(NCI CTCAE v5.0 ≥ 3 级)。
