背景:
音乐疗法广泛用于关系环境中。 声音可以影响边缘和旁边缘区域以及与运动密切相关的各种其他大脑区域(运动皮层、辅助运动区、小脑、基底神经节等)。 出于这个原因,音乐可以被认为是康复环境中的有用工具,特别是对于神经运动康复。
使用特定的基于音乐的技术可以引起从童年到老年的可塑性变化。 这些变化涉及大脑运动和听觉感觉运动区域,这要归功于声音和音乐引起的大脑区域之间连接的改善,如果没有听觉刺激就不会发生。 正如之前的研究表明的那样,音乐在大脑网络的节点中引起的可塑性变化可能会导致甚至在康复训练持续时间之后仍会持续存在的影响。 康复过程中的音乐也决定了情绪参与,并通过听觉刺激与感觉运动成分的耦合,创造了强大的动机基础,加强了其行动。

目标:

材料和方法:
该研究是一项多中心随机对照试验,将涉及 120 名临床稳定的中风患者 (n=40)、帕金森病 (n=40) 和多发性硬化症 (n=40)。 这三个手臂中的每一个都将分为两组:对照组 (n=20) 将进行标准运动康复,实验组 (n=20) 将进行相同的康复,但需要超声支持。 步态训练计划包括 20 节课,每节课 30 分钟,每周 3 次(详见下一章)。

实验性的:“声化”步态康复
声化康复练习由音乐成分支持(详见“干预”部分)。

声化系统由2个惯性传感器、一台电脑和一副与电脑相连的蓝牙耳机组成。
传感器将在每条腿的脚踝处放置一个,并与 Matlab 软件连接。
一个自制的专用软件将患者的动作与音乐模式联系起来。
在干预开始时检测并使用患者的自然节律。
每个练习的第一部分都由预先录制的和弦进行支持,只需单击背景即可。
在第二部分(超声处理方法)中,软件会注意并记录脚后跟与地面的接触。
每次接触都会激活通过耳机收听的音乐刺激。
步骤连续将建立与正确的步骤顺序相关的规则且可预测的音乐进程。
