回顾性分析了 2018 年 1 月至 2020 年 5 月期间在伊斯坦布尔梅德尼耶大学 Göztepe 培训和研究医院普通外科门诊接受 NAC 手术的 88 名乳腺癌患者。
记录了人口统计学数据、临床发现、肿瘤标志物、放射学发现和病理学数据。
使用 NAC 前后获得的 MRI 和/或 PET-CT,进行肿瘤分期,评估肿瘤大小和转移淋巴结的存在以及对 NAC 的放射学反应。
根据实体瘤反应评估标准 (RECIST) 和 Tozaki 等人描述的对比增强模式评估对 NAC 的反应。在 MRI 上,以及在 PET-CT 上的实体瘤分类 PET 反应标准 (PERCIST)。
术后病理报告如下:肿瘤的大小和组织病理学类型、激素受体状态、染色强度(Allred评分)、HER-2阳性、Ki-67增殖百分比和分子亚型;根据 Sataloff 分类,转移淋巴结的存在和数量,肿瘤和转移淋巴结(如果有)对 NAC 的反应。
Sataloff 得分为“A”的患者被认为具有 pCR,得分为“B”或“C”的患者被认为具有部分反应。
在检查 Sataloff 评分时,评估了原发肿瘤和腋窝中 pCR 的存在,并与 MRI 和/或 PET-CT 上放射学完全反应的存在进行了比较。
此外,根据分子亚型统计比较了 MRI 和 PET-CT 预测 NAC 患者 pCR 的敏感性。
