研究设计 该研究是一项基于前瞻性维护数据库数据的回顾性描述性登记研究。 数据招募包括在 2010 年 1 月 1 日至 2019 年 12 月 31 日期间接受手术且术后至少随访一年的所有 18 岁以上胰腺癌、腹水乳头和十二指肠腺癌患者。 研究完成日期为 2020 年 12 月 31 日。 患者分为两组,年龄小于 75 岁的患者和年龄大于 75 岁的患者。 此外,数据分为恶性肿瘤和非恶性肿瘤患者。 该研究根据 STROCSS 指南 (www.strocssguideline.com) 进行报告。
研究人群 该研究包括 1,556 名连续患者,1,296 名年龄在 18 至 75 岁之间的患者和 260 名年龄在 75 岁及以上的患者。 恶性肿瘤1208例,其中腺癌1020例。 其余 348 名患者有良性或无病理发现,但因癌前病变或怀疑恶性肿瘤而接受手术。
数据收集 数据是从我们前瞻性维护的胰腺手术数据库、电子医院记录系统 Orbit 和 EPIC、丹麦国家病理学数据登记处以及国家死亡登记处收集的。 所有丹麦国民都有一个唯一的中央个人登记号码,可以搜索健康数据。 来自法罗群岛和格陵兰岛的患者被排除在外,因为他们没有记录在丹麦的死亡登记册中。
结果 研究中记录了相关的术后并发症,包括胰腺、胆汁或胃空肠吻合术渗漏、腹腔内出血和脓肿形成或其他具有严重或致命后果的并发症。 结果定义为在 30 天和 90 天期间评估的术后并发症和死亡率,总生存期 (OS) 定义为从手术到任何原因死亡或在最后一次随访时审查的时间。 住院定义为术后住院直至出院。 住院死亡率定义为从入院到出院的所有死亡。 癌症特异性死亡率定义为在排除其他原因后死于腺癌。 记录的生命损失年定义为与年龄匹配的标准人口的预期剩余寿命相比的损失年。 如果患者没有发生缩短生命的事件,则记录的生命损失年数是本可以挽救的剥夺年数。
