肾癌占泌尿系统肿瘤的20.3%,且发病率仍在增加。 手术切除是肾癌的主要治疗方法;根治性肾切除术是T2期及以上肾癌的标准手术。 急性肾损伤(AKI)是根治性肾切除术后常见的并发症,发生率高达53.9%。 AKI 与慢性肾脏病 (CKD) 的发生有关,是无基础肾脏疾病患者新发 CKD 的独立危险因素。 一项荟萃分析显示,手术后一年,AKI 患者新发或进展 CKD 的风险增加 2.7 倍,患终末期肾病的风险增加 4.8 倍。 此外,即使是轻微的 AKI 也与术后 1 至 2 年的肾功能不全有关。

纳入标准:
- 年龄18岁或以上;
- 计划因肾癌接受根治性肾切除术。
排除标准
- 诊断患有慢性肾脏病第 4 期或第 5 期(GFR<30 ml/min/1.73m2) 手术前;
- 未控制的严重高血压(收缩压≥180mmHg或舒张压≥110mmHg);
- 合并心血管疾病,修正心脏风险指数 (RCRI) >1 或代谢当量 (MET) <4;
- 术前因严重痴呆、语言障碍或终末期疾病而无法沟通;
- 其他认为不适合纳入的情况(应注明具体原因)。

实验性的:有针对性的血压管理
麻醉期间,血流动力学管理包括主动补水(>10 ml/kg/h)、使用正性肌力药(多巴酚丁胺)和强制利尿;目标是维持脉压变化 <9%、平均动脉压 ≥85 mmHg 以及尿量 >200 ml/h (3ml/kg/h)。 在手术后的前 48 小时内,通过延迟抗高血压恢复、提供液体挑战和/或血管活性输注,将收缩压维持在 ≥110 mmHg 或基线的 20% 以内。 |
