背景
胆结石的存在是世界范围内非常常见的病症,尤其是在发达国家,据估计,胆结石影响着 10-15% 的成年人口,并占胃肠道相关医疗保健总支出的很大一部分[1]。 幸运的是,只有 10-25% 的胆结石在其一生中出现症状,年发病率为 2-3% [2, 3]。 此外,全球范围内肥胖和非酒精性脂肪肝 (NAFLD) 患病率的增加(NAFLD)是胆结石的危险因素,可能导致胆结石疾病患病率上升[4]。 胆囊收缩促进结石从胆囊迁移到胆总管(CBD)可被列为胆结石疾病的并发症。 在胆总管结石 (CBD) 迁移的情况下,应为适合手术的有症状和无症状患者提供内镜胰胆管造影 (ERCP) [1, 3]。 ERCP 结合充分的内镜下括约肌切开术可使约 80-90% 的患者获得完全的胆汁清除[5]。 虽然大多数病例可以通过此类一线方法成功治疗,但约 10-15% 需要替代和/或辅助技术来实现胆管清除[1, 5]。 这些情况通常被定义为“困难”胆管结石,这是一个广泛的病例类别,涵盖非常不同的情况,包括大块或多块结石、奇特的结石形状(例如桶形)、位于狭窄或受影响的结石上方、肝内结石结石、远端胆管改变(倾斜、变窄、乙状结肠)。 对于无法挽回的 CBD,欧洲胃肠内窥镜学会 (ESGE) 建议内镜下放置临时胆道塑料支架以保证胆道引流[1,5,6]。
大量的前瞻性和回顾性非随机研究表明,留置塑料支架可能会减少 CBD 的数量和大小[7-9],尽管内置假体有助于清除 CBD 的机制尚不清楚。 CBD 和支架之间的持续摩擦可能会产生机械应力,从而促进结石的破裂[10]。 现有数据显示,胆管塑料支架在既往ERCP后的取石成功率在44%~96%之间[11-14]。 自推出以来,全覆盖自膨式金属支架 (FCSEMS) 已迅速被用于治疗狭窄、渗漏或出血等良性胆道疾病。 最近的一项回顾性研究表明,FCSEMS 可用于治疗 CBD 的困难结石,并且没有相关的重大不良事件[9]。 然而,塑料支架和金属支架之间的比较前瞻性数据仍然缺乏。 理论上,与塑料支架相比,FC-SEMS 可以更有效地产生促进结石崩解的应力,但由于缺乏数据,该证据的质量仍然很低。
此外,具有特定假体设计(喇叭形 - Niti-S“S 型”Taewoong)的有前途的 FC-SEMS 可能更加有用,因为它减少了全覆盖支架的常见并发症,即假体的迁移[15 -17]。 然而,大多数有关 SEMS 迁移率的数据可用于治疗良性胆道狭窄,这是可以理解的,远端和近端迁移率均较低[17]。 然而,仍然缺乏关于使用 SEMS 清除不完全石材 CBD 的有效性和不良事件发生率的前瞻性数据。
ESGE 指南不建议进行明确的胆道支架置入术,但如果可能的话,有必要在第二次 ERCP 期间完全清除 CBD 结石。

- ≥ 18 岁且患有当前 ESGE 指南定义的困难胆总管结石的患者
- 无法挽回的胆道结石和胆道引流不完全的患者

- 胆道恶性梗阻
