术前运动对中高围手术期风险肺癌患者的短期影响(PRE-EXELUCA):一项多中心前瞻性随机对照临床试验
该临床试验的目的是比较不同强度的术前运动训练对计划进行肺切除术的患者的短期心肺功能和术后结果的影响。
研究概览
详细说明
广东省人民医院将作为牵头中心,另外 3 家医院将被指定为该研究的卫星中心。 综上所述,这些中心将总共招募100名同意的患者。 在知情同意和基线测试后,参与者将以 2:1:1 的比例随机分为三组之一:MICT、HIIT 或常规护理 (UC)。

纳入标准:
- 年龄>18岁。
- 疑似或确诊的原发性肺癌并计划进行手术治疗。
- 中至高围手术期风险,定义为 FEV1< 预测值的 60%(楔形)或 ppo FEV1< 预测值的 60%(肺叶切除/肺段切除)或 VO2peak<20 ml/kg/min。
- 签署知情同意书并同意遵守研究方案的要求。
- 完成基线完整心肺运动测试 (CPET)。
排除标准:
- 同时或既往有其他恶性肿瘤病史。
- 预期寿命不足1年。
- 失智;体弱多病;或无法参加运动(肌肉骨骼限制,即受伤、虚弱和虚弱)。
- 无法维持运动训练。
- 生命体征不规则(心率:静息时 < 40 或 > 120 bpm;血压:收缩压 > 200 mm Hg 或舒张压 > 110 mm Hg;T:≥ 38.5 或 ≤ 36°C;SpO2 ≤ 90%)。

实验性的:Moderate-intensity Continuous Training (MICT) Group
MICT plan is containing usual care protocol and additional complete a total of 12 supervised moderate-intensity continuous training sessions in 2 weeks.
|
Each MICT session consists of 30 minutes of cycling.
Workload will be gradually titrated from 50% to 60% peak WR throughout training to sustain a target RPE of 12-14 on the Borg 6-20 scale.

HIIT plan is containing usual care protocol and additional complete a total of 12 supervised high-intensity interval training training sessions in 2 weeks.

Each HIIT session consists of15 cycles, includes a 60-second high-intensity interval at 80-95% peak WR (target RPE 15-17) followed by a 60-second active recovery phase at 25% peak WR.
