상세 설명
광동성 인민병원이 주도 센터가 될 것이며, 추가로 3개 병원이 연구를 위한 위성 센터로 지정될 것입니다. 요약하면, 이들 센터는 총 100명의 동의한 환자를 모집할 것입니다. 사전 동의 및 기본 테스트에 따라 참가자는 2:1:1 비율로 MICT, HIIT 또는 일반 치료(UC)의 세 그룹 중 하나로 무작위 배정됩니다.
MICT 및 HIIT 그룹의 참가자는 총 12회의 감독 운동 훈련 세션을 수행하는 반면, UC 그룹에 무작위로 배정된 참가자는 추가 재활이나 신체 활동을 받지 않습니다. 모든 참가자는 중재 기간이 끝나면 추적 관찰됩니다. 심폐 운동 기능 및 폐 기능을 포함한 일차 결과는 진단(수술 치료가 예정된 폐암 환자)(V1), 기준선(V2) 및 중재 후 측정됩니다. (V3). 기본 평가에는 병력 및 검사가 포함됩니다. 환자의 연구에 대한 심리적 부담을 줄이기 위해 스크리닝 단계에서 검사가 완료되고 완료일이 등록 전 7일 이내인 경우 기본 검사 단계를 반복할 수 없습니다. 환자의 수술 후 합병증, 흉관 제거 시간 및 입원 기간(LOS)은 퇴원 당일(V4)에 기록됩니다. 환자 보고 결과(PRO)는 지속적인 추적 관찰과 함께 기준선(V2), 개입 후(V3), 퇴원일(V4), 수술 후 1개월(V5), 수술 후 3개월(V6)에 측정됩니다. 참가자의 혈액 샘플과 횡경막 초음파는 가능한 경우 기준선(V2) 및 중재 후(V3)에 보관됩니다. 수술 후 사망은 퇴원 당일(V4), 수술 후 3개월(V6), 수술 후 1년(V7), 수술 후 3년(V8), 수술 후 5년(V9)에 전화로 추적합니다.

설명
포함 기준:
- 연령>18세.
- 예정된 수술 치료를 통해 원발성 폐암이 의심되거나 확인된 경우.
- FEV1<예상 값의 60%(웨지) 또는 ppo FEV1<예측 값의 60%(엽절제술/분절절제술) 또는 VO2peak<20ml/kg/분으로 정의되는 중간에서 높은 수술 전후 위험.
- 사전 동의에 서명하고 연구 프로토콜의 요구 사항을 준수하는 데 동의합니다.
- 기준선 완료 심폐 운동 검사(CPET)를 완료합니다.
제외 기준:
- 다른 악성 종양의 동시 또는 이전 병력.
- 기대 수명은 1년 미만입니다.
- 백치; 허약; 또는 스포츠에 참여할 수 없음(근골격계 제한, 즉 부상, 허약함 및 허약함).
- 운동 훈련을 유지할 수 없음.

실험적: Moderate-intensity Continuous Training (MICT) Group
MICT plan is containing usual care protocol and additional complete a total of 12 supervised moderate-intensity continuous training sessions in 2 weeks.
|
Each MICT session consists of 30 minutes of cycling.
Workload will be gradually titrated from 50% to 60% peak WR throughout training to sustain a target RPE of 12-14 on the Borg 6-20 scale.

HIIT plan is containing usual care protocol and additional complete a total of 12 supervised high-intensity interval training training sessions in 2 weeks.

Each HIIT session consists of15 cycles, includes a 60-second high-intensity interval at 80-95% peak WR (target RPE 15-17) followed by a 60-second active recovery phase at 25% peak WR.
