先天性心脏病 (CHD) 是儿科疾病的重要组成部分,导致高发病率和死亡率。 由于心脏的胚胎学和冠状动脉的发育相互关联,CHD 患者的冠状动脉模式存在很大的变异性[1]。
冠状动脉异常的范围从良性/偶然发现到婴儿期显着的心脏病发病率或儿童或青少年期心源性猝死风险增加[2]。 冠状动脉的起源、走行、大小或血管数量可能存在异常。 在没有结构性心脏病的患者中,冠状动脉起源的孤立性异常的发生率估计为 0.7% [3]。 先天性心脏病儿童中冠状动脉异常的发生率较高,从缩窄患者的 5% [4] 到法洛四联症的 7% [5],单心室生理学患者的冠状动脉异常高达 37% [6] ]。 识别这些异常已成为术前评估的重要组成部分,以避免意外损伤,并且与手术可视化和导管血管造影相比,现代 CT 技术已被证明可以有效诊断 [6,7]。 冠状动脉起源和分支系统对于致力于先天性心脏病治疗的外科医生来说有很大的帮助[8]。

评估儿童冠状动脉的成像方式包括超声心动图、传统侵入性血管造影、磁共振成像 (MRI) 和计算机断层扫描 (CT)。 经胸超声心动图广泛用作主要成像方法。 然而,它由于其全面表征冠状动脉解剖的能力有限、声学窗口较差以及操作员依赖性而受到损害[10]。
磁共振成像 (MRI) 还提供有关心脏解剖结构和功能的信息,无需使用电离辐射即可进行 3 维冠状动脉成像。然而,它因采集时间长而受到损害,需要长时间的患者配合,而这在临床上可能是不可能的。儿童没有全身麻醉,并且空间分辨率有限,这使得冠状动脉评估超出原始评估变得困难[11]。
计算机断层扫描 (CT) 血管造影通常是成人和儿童冠状动脉无创成像的首选诊断方式。 CT采集速度快,可在多个支持设备的情况下进行。 与心脏 MRI 和经胸超声心动图相比,CT 提供了出色的空间分辨率以及冠状动脉整个走行的卓越可视化[12]。
CT 技术的最新进展使得辐射剂量显着降低,同时保持出色的空间分辨率,从而可以进行详细的解剖评估 [13]。
儿童冠状动脉异常可能是先天性的,也可能是后天性的。 先天性异常包括一组作为孤立病症或先天性心脏病谱系发生的复杂疾病。获得性冠状动脉异常主要继发于川崎病或先天性心脏病修复涉及冠状动脉操作时的手术。 越来越多的证据支持 CT 在这些患者的冠状动脉评估中的总体作用 [14,15]。 冠状动脉异常分为四种亚型,即起源和走行异常、冠状动脉固有异常、终止异常和吻合血管异常[16]。 冠状动脉异常在先天性心脏病患者中更为常见。 即使与临床无关,这些病变也可能变得很重要,因为它们可能影响手术修复。 因此,在这种情况下,必须进行明确的冠状动脉成像,特别是当超声心动图不能得出结论时,应采用 CT 以避免不必要的侵入性操作,即使在新生儿期也是如此。 在这种情况下要考虑的最常见情况包括法洛四联症和大动脉(右旋)转位[17]。

纳入标准:
- 1天至18岁患有先天性心脏病并接受手术矫正的儿童。
排除标准:
1-儿童有孤立的冠状动脉解剖异常。
2-儿童患有冠状动脉疾病,即 川崎病。
3-患有 CHD 且不适合进行手术矫正的儿童。
4-患有先心病且有心脏手术史的儿童。
