自我穴位按摩对改善头颈癌患者口干症的效果:一项随机对照试验
这是一项随机对照试验,旨在 1) 检查自我穴位按摩干预对患有口干症的头颈癌患者(主要结果)相对于口腔健康教育控制的影响; 2)检查自我穴位按摩对次要结果的影响,包括头颈癌患者的生活质量和症状严重程度; 3)评估患者对穴位按摩的期望; 4)探讨头颈癌口干患者自我穴位按摩的可接受性。
我们假设与口腔健康教育相比,自我穴位按摩对头颈癌患者的口干症有更好的效果。 穴位按摩还将有利于头颈癌患者的生活质量和症状严重程度。 头颈癌患者可能对自我穴位按摩有良好的接受度。

详细说明
口干症被定义为“口干的主观感觉”[1, 2]。 它在患有急性/慢性疾病的患者中很常见,尤其是癌症患者[3]。 观察性研究结果显示,癌症患者口干症的患病率为 40% 至 75% [4, 5]。 头颈癌患者作为第七种最常见的癌症类型 [6],与其他癌症类型相比,治疗后口干症的患病率甚至更高 (80%) [3,7,8]。 尽管口干症的患病率很高,但患者和医疗保健提供者常常对其认识不足。
头颈癌患者的口干症很大程度上是由头颈部区域的辐射引起的,辐射会损伤唾液腺,从而改变唾液的体积、稠度和 pH 值 [9]。 此外,同时或连续使用化疗药物和其他药物(例如阿片类药物)可能会引起或加剧口干症 [4, 10]。 癌症患者持续缺乏唾液会导致口腔真菌感染、龋齿、吞咽问题、睡眠问题、抑郁、疲劳[11]和味觉改变[12]的风险增加,甚至可能导致营养状况不佳[13]。 此外,口干症可能成为一种慢性甚至不可逆转的副作用[14]。
目前癌症患者口干症的治疗包括药物治疗和非药物治疗。 药物治疗被认为是刺激唾液分泌以减轻头颈癌患者口干症的一般治疗方法[15-17]。 然而,药物干预(例如毛果芸香碱、氨甲酰甲胆碱、氨磷汀、西维美林和帕利弗明)对口干症的证据不足[18],并且药物治疗的使用可能会引起一些常见的不良反应,包括恶心、出汗、紧张和尿频[15、16、18-20]。 非药物治疗,例如唾液兴奋剂(例如唾液刺激剂) 柠檬酸和苹果酸、口香糖、牙膏和含片)和唾液替代品(例如唾液替代品) 液体、凝胶和喷雾剂)也被建议用于治疗口干症[21],但都没有足够的证据表明显着效果[1,22,23],并且还可能导致不良反应(例如恶心、难闻的味道、腹泻、和牙齿矿化)[22, 24]。 有些患者可能会发现定期喝水很有用,但只是暂时的好处,因为唾液是一种复杂的物质,具有不可替代的功能(抗菌和免疫保护)[25]。 因此,对于伴有口干症的头颈癌患者,需要采取一种不良事件少的循证有效策略。
穴位按摩和针灸的目的都是通过刺激经络穴位来改善能量流动,从而改善健康和治疗疾病。 指压按摩是一种使用手、手指或拇指沿经络激活穴位的非侵入性技术,而针灸则涉及使用针[26]。 刺激穴位可以增加相关经络的气血运行,并刺激唾液腺周围的局部血流,从而增加唾液分泌[27]。 此外,指压/针灸可以刺激副交感神经系统,从而间接刺激唾液腺分泌[28]。 此外,还可以通过感觉神经肽的释放促进微循环,从而增加组织氧合和新陈代谢。 一些研究表明针灸对头颈癌患者的口干症有积极作用[29-35],但没有研究针压法。 基于相同的经络理论,针灸和穴位按摩都被广泛应用于症状管理(例如: 疲劳、恶心、疼痛)对癌症患者 [36, 37] 有效且具有良好的依从性 [38]。 与针灸相比,指压按摩可以由患者在训练后自行进行。 本研究的目的是检查自我穴位按摩对头颈癌患者口干症的影响。

描述
纳入标准:1)年龄≥18岁; 2)能够给予知情同意; 3) 诊断患有头颈癌(头颈部癌症,例如头颈癌) 口腔癌、鼻咽癌、口咽癌、下咽癌、喉癌、鼻窦癌、唾液腺癌); 4)完成至少2周的放射治疗或放化疗; 5) 治疗后主诉口干; 6) 能用粤语或普通话沟通。

排除标准:1)头颈癌治疗前有口干病史(例如干燥综合征); 2)最近3个月内进行过指压按摩或接受过针灸治疗; 3) 有针压禁忌症,例如 血液系统疾病(例如白血病);怀孕;哺乳期;上肢或下肢畸形;感染或穴位受伤。 过去一个月内以固定剂量方案服用药物/替代物质治疗口干症的人不会被排除在外。

实验性的:自我穴位按摩
参与者将参加由注册中医师举办的 90 分钟自我穴位按摩培训课程。
第二周将安排后续课程,以加强学习并确保所有参与者都能正确找到所建议的穴位。
后续会议将面对面进行(优先模式),或根据患者的喜好通过电话/视频通话进行。
根据文献综述 [25,29,34,35,39] 和 PI 的专家经验 [40-43] 选择纳入方案的穴位。
穴位(共9个)包括面颈部(任23、任24、ST5、ST6、ST7、TE17)、上肢(LI2、LI4)和下肢(KI6)。
除任二十三、任二十四穴外,其余穴位均应按摩双侧。
培训结束后,鼓励参与者在 12 周内每天两次自行对穴位进行指压,每个穴位持续 1 分钟。

客观测量口干症的严重程度将在上午 9 点至中午 12 点之间使用改良的 Schirmer 试纸进行测量。
改良的 Schirmer 试验是一种客观、耐受性良好的试验,用于测量头颈癌患者的口腔干燥程度。
在进行测试之前,所有参与者应禁食和饮水至少 15 分钟。
测量前将要求参与者吞咽一次。
将试纸插入口底三分钟(取出试纸两到三秒,分别在 1 分钟、2 分钟和 3 分钟读取试纸的读数),以测量未受刺激的唾液量,用一个-每次读数之间的分钟间隔)。
然后,用棉签将新鲜柠檬汁涂抹在舌侧两侧5次(分别在0、30秒、60秒、90秒和120秒时涂抹)。
