详细说明
为了调查中国IBD(炎症性肠病)患者对自身病情和远程医疗需求的认知程度,设计了一份调查问卷。 调查问卷包括IBD基础知识、治疗方法、日常管理、远程医疗需求等部分。 调查问卷内容将由专家团队讨论最终确定,然后通过微信客户端文卷行平台在全国多个中心分发,预计将有超过1000名患者参与。 此外,研究人员还将对长海医院的 50 名 IBD 患者进行半结构化访谈,以了解他们最关心的症状和问题。 这些访谈的结果将用于为患者制作教育材料。
构建并运营基于微信平台的IBD患者远程医疗管理系统,将采取以下步骤:
- 建立电子健康记录(EHR):每个登记的患者都会有一个包含个人信息、病史、疾病行为和治疗详细信息的 EHR。

描述
纳入标准:
- 患者将根据《炎症性肠病诊治共识(2018年,北京)》中概述的诊断标准诊断IBD。 结合临床症状、内镜检查结果、影像学检查结果和组织学检查结果来确定诊断。
- 参与者必须年龄在 18 岁至 75 岁之间,并签署该项目的知情同意书。
- 在入组前两年内,病历必须显示至少有一次被评估为炎症活动期(Mayo 评分 ≥ 3 或 CDAI 评分 ≥ 150)。
排除标准:
- 参与者被认为无法遵守研究人员确定的研究方案。
- 有 IBD 手术治疗史或计划接受手术的个人。
- 不受控制的内科或精神疾病。
- 在登记和随访期间怀孕或计划怀孕的人。

实验性的:接受远程医疗干预的患者
干预组参与者通过移动应用平台接受研究人员的远程医疗管理(建立电子健康档案、健康管理监测、风险预警与干预、健康教育)。

各参与中心研究人员根据患者的电子健康档案,一对一地对入组患者进行个性化用药提醒和随访提醒。
同时建议患者在家监测自身健康状况,定期在手机APP平台记录并上传临床症状、用药、饮食、运动等变化。
定期通过问卷星平台发放自我评估量表,评估并记录患者的疾病活动、生活质量、睡眠、心理状态等,并更新电子健康记录中的信息。根据患者的临床症状、自评量表以及医疗机构测试和检查的反馈,各参与中心研究人员将及时评估潜在的健康风险,发出预警警报,并提供有针对性的信息支持和干预措施。
