Inclusion Criteria:
- receipt of inpatient intravenous vancomycin during proposed study period
- adults 18 years of age or older and less than 90 years of age
Exclusion Criteria:
- individuals who are not yet adults (infants, children, teenagers)
- pregnant women
- prisoners
