Inclusion Criteria:
- All adults with uncontrolled HTN who receive care through iCHF membership and are able to provide informed consent.
Exclusion Criteria:
- Adults with controlled HTN or those without a diagnosis of HTN
- Unable or unwilling to provide informed consent
