Popis
Kritéria zahrnutí:
Věk ≥18 a ≤82 let
- Diagnóza: 1) diabetes 1. typu (po dobu nejméně 5 let) nebo 2) diabetes 2. typu
- Torontská konsenzuální kritéria budou použita pro definici případu DPN, kdy pacienti musí mít alespoň pravděpodobnou DPN [31]. Diagnóza DPN je potvrzena abnormální kontrolou DPN. Bolestivá DPN bude definována pomocí klasifikačního systému pro neuropatickou bolest (50) a bude v souladu s definicí neuropatické bolesti IASP, tj. „bolest způsobená lézí nebo onemocněním somatosenzorického systému“ (Torontská konsenzuální kritéria).
- TCNS skóre > 5
- Způsobilí pacienti s bolestivou DPN musí mít intenzitu bolesti alespoň 4 na 11bodové numerické hodnotící stupnici (NRS, 0-10) po dobu alespoň 3 měsíců na minimálně půldenní bázi a žádnou silnou bolest kromě bolesti způsobené neuropatie (intenzita bolesti bude založena na bolesti, kterou pacienti pociťují při současné léčbě bolesti, pokud existuje).
- Stabilní léky proti bolesti po dobu > 1 měsíce před zařazením. Kritéria vyloučení
Kritéria vyloučení:
- Současné neurologické (neurodegenerativní poruchy, migréna, epilepsie, mrtvice, nádor) nebo klinicky významné psychiatrické onemocnění
- Neuropatie nebo neuropatická bolest z jiných příčin než je diabetes (nedostatek vitaminu B12, předchozí léčba neurotoxickou chemoterapií, chronické zneužívání alkoholu, spinální stenóza atd.)
- Změna současné léčby bolesti během léčby (paracetamol je povolen jako záchranný lék)
- Předchozí nebo současné nadměrné užívání alkoholu (>14 nebo >21 jednotek/týden u žen a mužů) nebo zneužívání nelegálních látek
- Pozitivní výsledek testu hCG v moči svědčící o těhotenství
- Užívání morfia >20 mg/den
- Slepota nebo vážně zhoršené vidění
- Zkoušející zjistí, že pacient není vhodný pro studii (např. v důsledku požívání alkoholu nebo drog, mentální neschopnosti, neochoty nebo jazykové bariéry znemožňující adekvátní porozumění nebo spolupráci nebo přítomnost jakéhokoli stavu, který podle názoru výzkumníků může vést ke špatnému dodržování protokolu studie).

Experimentální: Skutečný EEG-neurofeedback

Traditional neurofeedback uses one or two electrodes to modulate activity within a specific frequency band.
Standardized Weighted Low Resolution Electromagnetic Tomography (swLORETA) analyzes the 3D distribution of intracortical brain electrical activity based on surface EEG recordings, enabling real-time brainwave imaging with a spatial resolution under one cubic centimeter.
This divides the brain into over 12,000 voxels, offering localization similar to fMRI while maintaining EEG's faster temporal resolution.
Source-localized NF can target specific, deeper brain regions, multiple Brodmann areas simultaneously, and provide feedback on connectivity between neural sources, enabling the training of specific neural networks.
swLORETA metrics are compared to a normative database of neurotypical brains to produce z-scores for each area and metric.
NeuroGuide is used within the FDA 510(k)-cleared NeuroGuide Analysis System (K041263); clearance does not imply treatment validation.

The primary aim is to evaluate whether real EEG-NF compared with sham EEG-NF, leads to a greater reduction in self-reported pain intensity (NRS from 0-10) from T0 (baseline) to T1 (after 10th and final session), assessed using mean 7-day pain intensity derived from an electronic pain diary.

Pain network abnormality burden

Neurophysiological outcomes will be assessed using normative z-scores within the predefined pain-related network.
A composite pain-network abnormality burden will be derived across EEG features, with domain-specific measures for current source density (CSD), instantaneous coherence, and lagged coherence also examined.
Changes from pre- to post-intervention will be compared between the real EEG-NF and sham EEG-NF groups.

Global impression of change
