Beschreibung
Einschlusskriterien:
Alter ≥18 und ≤82 Jahre
- Diagnose: 1) Typ-1-Diabetes (seit mindestens 5 Jahren) oder 2) Typ-2-Diabetes
- Die Toronto-Konsenskriterien werden für eine Falldefinition von DPN verwendet, bei der Patienten mindestens eine wahrscheinliche DPN haben müssen (31). Die DPN-Diagnose wird durch einen abnormalen DPN-Check bestätigt. Schmerzhaftes DPN wird anhand des Bewertungssystems für neuropathische Schmerzen (50) definiert und entspricht der IASP-Definition von neuropathischem Schmerz, d. h. „Schmerz, der durch eine Läsion oder Erkrankung des somatosensorischen Systems verursacht wird“ (Konsenskriterien von Toronto).
- TCNS-Score > 5
- Geeignete Patienten mit schmerzhafter DPN müssen eine Schmerzintensität von mindestens 4 auf einer 11-stufigen numerischen Bewertungsskala (NRS, 0-10) für mindestens 3 Monate mindestens halbtäglich und keine starken Schmerzen außer Schmerzen aufgrund von haben Neuropathie (die Schmerzintensität basiert auf den Schmerzen, die die Patienten gegebenenfalls während der aktuellen Schmerzbehandlung verspüren).
- Stabile Schmerzmedikation für > 1 Monat vor der Aufnahme. Ausschlusskriterien
Ausschlusskriterien:
- Begleitende neurologische (neurodegenerative Erkrankungen, Migräne, Epilepsie, Schlaganfall, Tumor) oder klinisch bedeutsame psychiatrische Erkrankung
- Neuropathie oder neuropathischer Schmerz aufgrund anderer Ursachen als Diabetes (Vitamin-B12-Mangel, vorherige Behandlung mit neurotoxischer Chemotherapie, chronischer Alkoholmissbrauch, Spinalkanalstenose usw.)
- Änderung der aktuellen Schmerzbehandlung während der Behandlung (Paracetamol ist als Notfallmedikament zulässig)
- Vorheriger oder aktueller übermäßiger Alkoholkonsum (>14 bzw. >21 Einheiten/Woche für Frauen bzw. Männer) oder illegaler Drogenmissbrauch
- Ein positives hCG-Testergebnis im Urin weist auf eine Schwangerschaft hin
- Morphinkonsum >20 mg/Tag
- Blindheit oder stark beeinträchtigtes Sehvermögen
- Der Prüfer stellt fest, dass der Patient für die Studie ungeeignet ist (z. B. aufgrund von Alkohol- oder Drogenkonsum, geistiger Unfähigkeit, Unwilligkeit oder Sprachbarriere, die ein angemessenes Verständnis oder eine angemessene Zusammenarbeit oder das Vorliegen einer Erkrankung ausschließen, die nach Meinung der Prüfer zu einer mangelhaften Einhaltung des Studienprotokolls führen kann).

Experimental: Echtes EEG-Neurofeedback

Traditional neurofeedback uses one or two electrodes to modulate activity within a specific frequency band.
Standardized Weighted Low Resolution Electromagnetic Tomography (swLORETA) analyzes the 3D distribution of intracortical brain electrical activity based on surface EEG recordings, enabling real-time brainwave imaging with a spatial resolution under one cubic centimeter.
This divides the brain into over 12,000 voxels, offering localization similar to fMRI while maintaining EEG's faster temporal resolution.
Source-localized NF can target specific, deeper brain regions, multiple Brodmann areas simultaneously, and provide feedback on connectivity between neural sources, enabling the training of specific neural networks.
swLORETA metrics are compared to a normative database of neurotypical brains to produce z-scores for each area and metric.
NeuroGuide is used within the FDA 510(k)-cleared NeuroGuide Analysis System (K041263); clearance does not imply treatment validation.

Schein-Komparator: Schein-EEG-Neurofeedback

Pain intensity

The primary aim is to evaluate whether real EEG-NF compared with sham EEG-NF, leads to a greater reduction in self-reported pain intensity (NRS from 0-10) from T0 (baseline) to T1 (after 10th and final session), assessed using mean 7-day pain intensity derived from an electronic pain diary.

Neurophysiological outcomes will be assessed using normative z-scores within the predefined pain-related network.
A composite pain-network abnormality burden will be derived across EEG features, with domain-specific measures for current source density (CSD), instantaneous coherence, and lagged coherence also examined.
Changes from pre- to post-intervention will be compared between the real EEG-NF and sham EEG-NF groups.

Global impression of change
