Zkoumání účinků terapeutického zaměření na pozitivní vzpomínky na posttraumatické zdraví
Vyšetření a zdokonalení nové léčby posttraumatické stresové poruchy: Technika zpracování pozitivních vzpomínek (PPMT)
Posttraumatická stresová porucha (PTSD) má zničující zdravotní následky. Intervence PTSD založené na důkazech se zabývají podstatnou zátěží PTSD na zdraví jednotlivců a společností; nicméně několik jedinců, kteří podstoupili tyto intervence, odstoupilo a ne u všech jedinců došlo ke zlepšení symptomů PTSD. Navíc se tyto současné intervence PTSD primárně zaměřují na traumatické vzpomínky. Rostoucí důkazy naznačují, že symptomy PTSD souvisejí také s obtížemi při kódování a získávání pozitivních vzpomínek.
Navrhovaná studie tedy bude zkoumat účinky a cíle, které jsou základem nové techniky PTSD zaměřené na vyprávění a detailní popis pozitivních vzpomínek – Technika zpracování pozitivních vzpomínek (PPMT). Metodicky bude pro tuto studii náhodně rozděleno 70 jedinců do PPMT vs. podpůrného poradenství (SC).
Cíle navrhované studie zahrnují (1) zkoumání účinků PPMT na závažnost symptomů PTSD a dysregulaci stresových systémů (tj. probuzení slinné alfa amylázy [sAA] a kortizolu); (2) zkoumání mechanismů, které jsou základem účinků PPMT; a (3) rafinace PPMT. Předpokládá se, že rameno PPMT bude hlásit větší poklesy závažnosti PTSD a poměrů sAA/kortizol. Dále se předpokládá, že zlepšený účinek související s PPMT bude zprostředkovávat spojení mezi ramenem studie (PPMT vs. SC) a změnami v závažnosti PTSD. A konečně, od účastníků studie bude získána zpětná vazba o proveditelnosti, formátu a obsahu PPMT pro upřesnění PPMT.
Navrhovaná studie může přispět k předběžným důkazům o potenciálním významu zacílení pozitivních vzpomínek při intervencích PTSD.

- Ochota/schopna poskytnout informovaný souhlas
- Diagnóza posttraumatické stresové poruchy (PTSD) za poslední měsíc DSM-5
- Pracovní znalost angličtiny
Kritéria vyloučení:
- Aktivní sebevražedné úmysly nebo pokusy
- Aktivní psychóza
- V současné době podstupujete terapii duševního zdraví (poslední 3 měsíce)
- Změny dávkování psychiatrických léků (za poslední 3 měsíce)
- Momentálně těhotná

Aktivní komparátor: Technika zpracování pozitivních vzpomínek (PPMT)
Účastníci absolvují 5 60minutových sezení techniky zpracování pozitivních vzpomínek (PPMT) pro PTSD podle léčebných postupů, jak je uvedeno v terapeutické příručce PPMT.

PPMT se skládá z 5 sezení zaměřených na identifikaci a zpracování pozitivních vzpomínek.
V relaci 1-5 účastníci vyprávějí podrobnosti o jedné pozitivní vzpomínce a zpracovávají pozitivní vzpomínku, aby vyvolali pozitivní hodnoty, vliv, silné stránky a myšlenky.

Aktivní komparátor: Podpůrné poradenství (SC)
Účastníci absolvují 5 60minutových sezení podpůrného poradenství (SC).
SC zahrnuje diskusi o denním monitorovacím deníku a zkoumání aktuálních problémů a obav.

Awakening Salivary Alpha Amylase to Cortisol Ratio

Via a passive drool procedure, participants provided 3 whole saliva samples each day for 2 consecutive days both prior to PPMT/SC: (1) immediately upon awakening, (2) 30 minutes post-awakening, and (3) 60 minutes post-awakening.
For each marker, for each day, area under the curve with respect to ground (AUCg) was calculated.
The two-day average AUCg of sAA was then divided by the two-day average AUCg of cortisol to create the ratio variable.
Higher sAA/cortisol ratio indicates greater stress system dysregulation and sympathetic nervous system dominance.
